Community aftercare team provides ongoing case management and support for youth for a minimum of one year to ensure positive reintegration.

An innovative outpatient treatment program for adolescents and young adults who may be experiencing mental health issues. OnTrackNY helps these individuals achieve their goals for school, work and relationships. Individuals will have access to a Psychiatrist, Nurse, Therapist, Peer Specialist and Education and Employment Specialist when enrolled in OnTrackNY.

Provides in-home support services and activities to allow intellectually and/or developmentally disabled persons to live, work, socialize and participate in the community.

Provides case management and coordinated support services, which include advocacy, referral and resources through the Home and Community Based Medicaid Waiver program, to developmentally disabled individuals 18 years or older who are medicaid eligible and meet criteria for participation in OPWDD programs.

Provides help for individuals with Multiple sclerosis in getting services, assessing the needs, case management, and guiding them to programs and services in the community.

Assists clients who are on Medicaid who has two or more chronic conditions, or one active mental health diagnosis, or HIV/AIDS. The program's goal is to improve clients' quality of life while keeping them engaged in medical care.

Fundamental to the Community Independence Program is the belief that individuals with developmental disabilities can and should be given the opportunity to determine the content and character of the service they receive, identify the goals to which they aspire, and choose the individuals that would provide them with assistance in identifying and attaining their goals.

Assists individuals with developmental disabilities and their families in gaining access to services and support for their needs. Provides service coordinators who use a person-centered planning process in developing, implementing and maintaining an Individualized Service Plan (ISP).

Provides assistance to individuals with mental health issues. Program will assist them in their own recovery process. Direct assistance and linkages provided.

Provides services for adults and children with a mental health diagnosis and/or chronic medical conditions, including linkage to needed resources and coordination of services to assist clients with reaching their goals.

Provides care coordination to children who have serious emotional disturbances and/or complex medical needs.

Provides case management and prenatal/postnatal home visiting services. Utilizes trained Community Health Workers (CHW) to identify, recruit, and assess the needs of targeted women before, during, and after their pregnancy. CHWs provide women with referrals for services needed, education regarding the benefits of family planning and early prenatal care, assistance with obtaining health insurance and medical care, and follow-up to assure that services were secured. Additional services include breastfeeding support and a smoking cessation program.

Assists individuals with developmental disabilities and their families to review concerns, then develop an ISP (Individualized Service Plan) then help them gain access to the necessary services and supports appropriate to the needs of the individual.

Provides services for children, youth, adults and seniors, such as drinking driver program, educational and counseling services, Nursing Home Transition and Diversion Program (NHTD), Traumatic Brain Injury (TBI), as well as a variety of services to individuals with developmental disabilities.

DBCH provides individual case management and follow-up to infants born to mothers who are hepatitis B positive.

Provides case management and non-medical in-home services to income-eligible seniors 60 and over. Potential clients cannot be receiving assistance from Medicaid or any other entitlement program.

Promotes community integration and independence for developmentally disabled adults and adults with epilepsy and MR. Participants visit volunteer sites, engage in recreation, and learn socialization and independent life skills including banking, time management, shopping and personal care. Provides HIV prevention interventions, HIV testing, HIV screening, care management for chronic illness, syringe exchange program, opioid overdose prevention training.

Gurwin Certified Home Health Agency provides part-time, intermittent skilled services care of a therapeutic, rehabilitative, and restorative nature to persons in their home. Our team of clinicians provides skilled nursing care, physical/occupational/speech therapy and social work services.

Offer long-term home health care featuring the following: nursing care, home visits, case management, physical and occupational therapy, speech and hearing services, social work services, personal care, homemaking and housekeeping services, home health aide, nutritional counseling, respiratory therapy, medical supplies and equipment and medical transportation. Waivered services include personal emergency response system, housing improvement, home maintenance, social day care and transportation and respite care.
