Children's health home program providing case management services for children ages 0-21 years old. Helps connect to social and medial supports in the community and provides assistance with Department of Social Services and existing community resources.
A community-based support service for adults throughout Erie County with serious and persistent mental illness and adults with Medicaid that have two chronic health conditions. Provides individual service planning with the following services available to those receiving services: crisis intervention on a 24-hour basis; linkages to medical, psychiatric, social, educational, psychological, and vocational services within the community; coupled with individual and systemic advocacy.

Focuses on creating asset development opportunities for both residents and entrepreneurs. Assists individuals in growing and investing in their communities by providing lending capital, training, consulting, technical assistance, seminars, workshops, and low-cost business rental space to new and existing businesses that would otherwise lack access to these services. Assists area residents in developing asset based thinking by providing financial literacy training, basic budgeting, and home appliance loans to combat predatory lenders. Loan programs and Cottage Industry Assistance Program target low to moderate income existing and prospective individuals. Chautauqua Microenterprise Loan Fund and the Women and Minority Microenterprise Loan Fund Programs help small businesses by providing small business start up and expansion loans. The small business incubator offers small businesses an opportunity to grow and stabilize with subsidies around rent, business services, and technical assistance. Program site serves as a Warming Center during times of excessive cold and as a Cooling Center during times of excessive heat. Always call ahead to make sure a Warming/Cooling Center is open before going there.

The Childhood Lead Poisoning Prevention Program (Child Lead) is responsible for the case management of lead poisoned children in Erie County. Addresses the potential for lead exposure, as well as preventing lead poisoned children from further exposure. Conducts investigations and provides information to the parents or guardians of children under the age of 18 that have tested positive for elevated blood lead level. New York State Guidelines require that all children be screened for lead at ages one and two years and every year up to age six if certain risk factors, such as residing in pre-1978 housing, are present. Activities of Child Lead include: - Case management for children with elevated blood lead levels - Providing information on legislation, sources of lead exposure, methods to reduce lead exposure, medical treatment, and lead hazard reduction activities - Leadership and coordination of lead activities in the community - Lead testing for children under age 6
Rape Crisis Center that also provides services addressing Domestic Violence, Family Violence, and Elder Abuse. Provides 24-hour immediate crisis response to all Erie County Hospital Emergency Departments when a victim of rape, domestic/family violence, or elder abuse is identified. Services include crisis intervention, support, advocacy, safety planning, case management, supportive counseling, prevention education, and community referrals. Crisis Services also has a Sexual Assault Nurse Examiner Program that operates throughout Erie County and Niagara County in partnership with YMCA Niagara. Satellite offices are located at the Buffalo Police Sex Offense Section, Town of Tonawanda Police Department, and Erie County Sheriff's Office. Speakers and trainers are available. Advocates are also co-located at various college campuses across Erie County.
