Children's health home program providing case management services for children ages 0-21 years old. Helps connect to social and medial supports in the community and provides assistance with Department of Social Services and existing community resources.

A community-based support service for adults throughout Erie County with serious and persistent mental illness and adults with Medicaid that have two chronic health conditions. Provides individual service planning with the following services available to those receiving services: crisis intervention on a 24-hour basis; linkages to medical, psychiatric, social, educational, psychological, and vocational services within the community; coupled with individual and systemic advocacy.

Focuses on creating asset development opportunities for both residents and entrepreneurs. Assists individuals in growing and investing in their communities by providing lending capital, training, consulting, technical assistance, seminars, workshops, and low-cost business rental space to new and existing businesses that would otherwise lack access to these services. Assists area residents in developing asset based thinking by providing financial literacy training, basic budgeting, and home appliance loans to combat predatory lenders. Loan programs and Cottage Industry Assistance Program target low to moderate income existing and prospective individuals. Chautauqua Microenterprise Loan Fund and the Women and Minority Microenterprise Loan Fund Programs help small businesses by providing small business start up and expansion loans. The small business incubator offers small businesses an opportunity to grow and stabilize with subsidies around rent, business services, and technical assistance. Program site serves as a Warming Center during times of excessive cold and as a Cooling Center during times of excessive heat. Provides care coordination, specialty and primary health care, HIV/AIDS specialty care, LGBTQ+ specialty care, STI/HCV testing, syringe exchange, buprenorphine program, and harm reduction program. Also provides linkage to mental health services, substance use counseling, housing assistance, nutrition assistance, and transportation assistance.

The Environmental Health Division is dedicated to protecting the citizens and visitors of Erie County from biological, chemical, and physical hazards that may be present in food, water, and the built environment. Environmental Health sanitarians, engineers, nurses, educators and vector control workers: - Conduct inspections and investigations - Educate and motivate the public to prevent environmental hazards - Provide medical case management for children diagnosed with lead poisoning - Provide timely environmental health information in times of emergency - Provide assistance and information in cases of potential rabies exposures - Review plans and advise the public in environmentally safe construction, development and upgrades to facilities - Assist in the control of disease-causing vectors - Enforce applicable local, state, and national codes, and regulations to protect public health

Provides casework services in the fields of Child Protection, Adult Protection, Preventive Services, Foster Care, Kinship Care, Adoption, and Domestic Violence. Caseworkers assist families in dealing with complex human problems that impair or prevent them from achieving or maintaining independence.

The Childhood Lead Poisoning Prevention Program (Child Lead) is responsible for the case management of lead poisoned children in Erie County. Addresses the potential for lead exposure, as well as preventing lead poisoned children from further exposure. Conducts investigations and provides information to the parents or guardians of children under the age of 18 that have tested positive for elevated blood lead level. New York State Guidelines require that all children be screened for lead at ages one and two years and every year up to age six if certain risk factors, such as residing in pre-1978 housing, are present. Activities of Child Lead include: - Case management for children with elevated blood lead levels - Providing information on legislation, sources of lead exposure, methods to reduce lead exposure, medical treatment, and lead hazard reduction activities - Leadership and coordination of lead activities in the community - Lead testing for children under age 6

Rape Crisis Center that also provides services addressing Domestic Violence, Family Violence, and Elder Abuse. Provides 24-hour immediate crisis response to all Erie County Hospital Emergency Departments when a victim of rape, domestic/family violence, or elder abuse is identified. Services include crisis intervention, support, advocacy, safety planning, case management, supportive counseling, prevention education, and community referrals. Crisis Services also has a Sexual Assault Nurse Examiner Program that operates throughout Erie County and Niagara County in partnership with YMCA Niagara. Satellite offices are located at the Buffalo Police Sex Offense Section, Town of Tonawanda Police Department, and Erie County Sheriff's Office. Speakers and trainers are available. Advocates are also co-located at various college campuses across Erie County.

Care Managers help identify and link clients and their families to services that can help meet their medical, social/emotional, behavioral, and basic needs. Clients are youth up to 21 years of age with active Medicaid, diagnosed with chronic physical and/or mental health conditions, HIV/AIDS, or Complex Trauma.

Provides comprehensive assessments of an individual's home health care needs, coordinates the most appropriate effective services, and provides case management. Designed to help health impaired Erie County residents remain in their homes. Provides linkage for clients with home care needs including personal care, meals, and housekeeping. Screens for Medicaid eligibility for home care.

Provides a variety of support services for families with a child diagnosed with autism, including care management, and are at different stages of their life. Helps parents who are learning to cope with autism for the first time, or parents whose children are older, for siblings, grandparents, and caregivers.

A family shelter for homeless women and children providing temporary emergency housing and case management.

Offers service coordination for individuals with intellectual and/or developmental disabilities, their families, and caregivers. Coordinators assist individuals to enroll in programs and access needed services. Coordinators act as an advocate as well as a source of support. Documentation of developmental disability required.

Community residence for adolescent males and females who are in need of a structured setting in the community that enables them to become involved in the responsibilities of daily living due to a serious emotional disturbance (SED).
