Provides a mandatory enrollment program that works with other military and civilian agencies to provide comprehensive and coordinated medical, educational, housing, community support, personal services, and advocacy assistance to families with special needs.

Provides case management services to children with serious mental disturbance or chronic health conditions to assist them in staying healthy, out of the emergency room, and out of the hospital.

Coordinates services for individuals with traumatic brain injuries, as well as any other illness or disability that will possibly place a person in a nursing home.

Provides advocacy, assistance, and role modeling for high risk homeless youth with a mental health diagnosis ages 18 to 24 years. Program options include case management, housing placement, employment assistance, and emergency basic needs.

* Provides employment and training opportunities to adults, youth, and dislocated workers. Also provided are job search assistance, vocational planning, labor market information, career counseling, and case management. * Offers workshops on such topics as Resume Writing, Cover Letter Writing, Interviewing, and Computers. * Offers training programs based on needs of the community and market. Types of training may include: LPN (Licensed Practical Nurse) training. CNA (Certified Nurse Assistant) training. CDL (Commercial Driver's License) training.

* Works with high-need women and families to access preventive and primary health care services, with an emphasis on pregnant women who are not enrolled in health care or supportive services.

Provides a care coordination program that assists individuals that are residing in a community setting or are transitioning to live in a community setting. Services include behavioral, educational, and social health support, transportation, and housing support. On-call services are available 24/7.

Provides testing, pre/post counseling, treatment, social services and nutritional services for anyone with AIDS/HIV.

A high end intensive case management program providing wrap around services for at risk youth and their family. Serves as an alternative to out of home placement for youth. Based upon his or her individual needs, each youth who participates in the program receives a combination of services.

Assists clients who are on Medicaid who has two or more chronic conditions, or one active mental health diagnosis, or HIV/AIDS. The program's goal is to improve clients' quality of life while keeping them engaged in medical care.

Provides a myriad of supportive services to very low-income Veteran families who are residing in permanent housing, are homeless and scheduled to become residents of permanent housing within a specified time period, or after exiting permanent housing, are seeking other housing that is responsive to such very low-income veteran family's needs and preferences. Support services include personal financial planning services, income support services, legal services, housing search and counseling, vocational and rehabilitation services through the New York State Department of Labor, employment training through the Suffolk County One-Stop Employment Center, outreach services, peer support, case management services, assistance obtaining VA benefits and other public benefits.

Provides a support program for people living with chronic health conditions. Helps individuals get the treatment and support to stay healthy and out of the emergency room and hospital.

This is a private care management program providing compassionate service to clients and their families- comprehensive in home assessment, care planning, care management, eldercare consulting, assistance with housing, facility placement, entitlements, advocacy, and education.

* Provides an outpatient community mental health clinic treatment services for children and youth that includes: Intake assessment & evaluation. Case management, supportive and intensive. Crisis services. Individual, group, and family therapies. Skill-building interventions for symptom management. Referrals as indicated. Satellite clinics in all Wayne County school districts. Child Psychiatric Services.

The Case Management program is designed to serve severely and persistently mentally ill and seriously emotionally disturbed adults. This 24-hour-a-day program provides aggressive outreach and support to individuals who either overuse emergency room and inpatient services or drop out of services. Access is by referral.
