Latinas Prosperando or Thriving Latinas program is for Latina immigrant women who are survivors of domestic violence, sexual assault and/or stalking. Case management, ESL classes, financial literacy, education on housing search options, and workforce development support is provided. Knitting Our Stories - Tejiendo Nuestras Historias, provides a safe space where women can learn to heal from past traumatic experiences through a culturally-adapted support group and art therapy.

Provides services under the Department of Health Traumatic Brain Injury Waiver Program. This program assists individuals obtain in and out of home supports to maintain their optimal level of independence. Services provided include: service coordination, structured day program, community integration counseling and behavioral supports for individuals diagnosed with traumatic brain injury

Offers In-home services and supports funded under the Expanded In-home Services for the Elderly Program (EISEP) and administered by Nassau County Office for the Aging and its subcontractor agencies. Care coordination is a person-centered planning process that takes place between a qualified case manager/care coordinator, the applicant, and, often, members of the applicant’s informal support network. Care coordination begins with a comprehensive assessment that identifies an individual’s needs, abilities, preferences and available resources. Next, a care plan is developed based on the findings of the assessment. The care plan is a coordinated package of services and supports derived from multiple sources to best meet the applicant’s needs. Finally, ongoing monitoring helps to ensure that the plan is adequate and the program participant is meeting his/her goals and is residing safely in the community. Call for detailed information.

Offers a variety of home care services for Medicaid recipients who are elderly, infirmed or disabled. Some services include personal care, case and care management, personal emergency response devices, long-term home care for individuals who are eligible for a nursing home, but choose to stay at home. Home care services are physician ordered and supervised by a registered nurse, and services are rendered by licensed providers who are contracted and monitored the Dept. of Social Services.

Provides targeted outreach and HIV prevention education, case management services, support groups, and counseling services to HIV infected individuals. Provides health services to individuals who are currently incarcerated. Services include outreach, education, and testing to prevent the spread of HIV, Hepatitis C, and STIs, and assists individuals living with HIV and Hepatitis to develop a care plan once they're discharged back to the community.

Provides case management services to children with serious mental disturbance or chronic health conditions to assist them in staying healthy, out of the emergency room, and out of the hospital.

Offers services for individuals from birth to 21 years of age who have or are suspected of having a serious or chronic condition and require health and related services of a type or amount beyond typically required by children. Assists with insurance access, setting up diagnostic testing and evaluations, offsetting medicals bills, and paying for hearing aids and other equipment.

Wildwood Programs' Medicaid Service Coordination assists individuals with developmental disabilities to gain access to the full array of services and supports available to them in the community, while promoting the concepts of choice, individualized services and supports and consumer satisfaction. The service coordinator works collaboratively with the individual, family and other service providers to develop and implement an individualized service plan, which outlines a person's goals, desires, and dreams.

Residential services offers program where individual 18+ yrs can maximize his/her potential while remaining in one's community.
