Care coordination begins with a comprehensive assessment that identifies an individual’s needs, abilities, preferences and available resources. Next, a care plan is developed based on the findings of the assessment. The care plan is a coordinated package of services and supports derived from multiple sources to best meet the applicant’s needs.
