The House of The Good Shepherd provides Health Home care coordination services which include; comprehensive care management services, care coordination and health promotion services, comprehensive transitional services, individual and family support services, as well as a referral services for both community and social support services. Every eligible client is provided a care manager who is the primary contact for all things related to the client's care and services. The assigned care manager works to advocate for the client and ensure that all members of the client's care team are working together to make progress on the client's goals that will benefit and improve their life (mental, physical, behavioral, and social).

Provides a PINS diversion program. AFY is a 30-45 day home based crisis intervention for youth that display a pattern of incorrigible behavior(Defiance in the home, disobeying curfew, aggressive behaviors, truancy/failing school and mental health issues). Case Managers will do an assessment of family needs and links them with community resources to get the family out of crisis. In addition, after the 30-45 days are completed. a Phase 2 of the program is implemented to link the families with partnership agencies for typically 3-6 months, but can be up to a year to ensure the ensure the family's needs are met and they remain stabilized. AFY also provides parent support workshops, adolescent male and female workshops as well as peer mentoring services.

Case Management, screening, assessment, care planning, determining eligibility for program subsidies, arranging and authorizing service delivery, services follow-up and monitoring. In-Home Services including housekeeping and chores, homemaking and personal care Personal Emergency Response Systems (PERS) units, which assist people to live active, independent lives by linking them to year-round assistance 24 hours a day. A PERS unit can be used for fire, police, and medical emergencies. Social Adult Daycare Services (SADS), a planned program of activities in a professional care setting designed for older adults who require supervised care during the day.
* Provides assesment and guidance for children, youth and families who are at risk of out-of-home placement. Referals to mental health and subtance abuse resources. Confidential coordination of community base service providers to assist the child and family. Child and family are welcome to attend the coordination meeting.
