They help people avoid hospitalization and achieve their goals by linking them to necessary services in the community including treatment, entitlements, housing, education and vocational development.

Eligibility

Must have history and current diagnosis of serious mental illness, chemical addiction or two or more chronic physical illnesses. Must be eligible for Medicaid. Must live in Suffolk County.

Works to improve services to people who have mental health, substance abuse and/or medical challenges both in institutions and in the community through education, information and referral services, legislative action and program development.
