Provides services for the diagnosis and care of Alzheimer's disease patients. Functions as a comprehensive resource to the healthcare community and provides diagnostic and consultative services to primary care providers in making initial assessment of patients, including the development of comprehensive care plans, assistance with the management of complex patient situations and referrals of patients to adjunct specialty care. Provides diagnosis and assessment for all persons suspected of having dementia. Offers patient management and care for those who are diagnosed with Alzheimer's disease and other forms of dementia and require an appropriate care management plan. Provides training and continuing education for medical professionals, education and training to family caregivers, community organizations, and support groups for caregivers. Serves as a regional information resource center for health and human service providers, caregivers of patients with Alzheimer's disease and other dementias and the general public.
