Cancer nurse navigators: guide patients and families through cancer care: screening, diagnosis, treatment and support services act as patient advocates, interpreters and educators serve as liaisons between patients, caregivers and service providers

* Provides Health Home services through a group of health care and service providers. Enrolled individuals will work with a care manager to develop an individualized care plan.* Assists individuals with: Coordinating care through communication with other providers. Advocating on their behalf to receive care. Navigating the healthcare system. Connecting to mental health and substance abuse providers. Connecting to needed medications. Linking to resources for finding affordable housing, benefits, food, clothing, childcare, transportation. Learning more about activities to improve health. Identify goals and ensure all providers involved in care understand the goals and care plan.

Provides assistance to children from birth to 21 years of age who have (or may have) a serious or chronic physical, developmental, behavioral, or emotional condition in locating a physician or other health care provider and may also provide assistance in obtaining insurance and/or other programs that help to cover health care costs. Supportive measures as well as medical information may be provided.

Facilitates a smooth transition from the hospital to home for Nassau County children under 18 years of age with mental illness. Provides crisis assessment, individualized planning and family needs assessments. Offers family support by a treatment team to ensure adequate discharge planning and access to community-based services to promote long term symptom management and recovery.

Making Headway Foundation provides supportive services to families who have a child with a brain or spinal cord tumor.

Provides inpatient care to the terminally ill at Hospice House. Care includes: Therapists, Registered Nurses, Clinical Dieticians, Pastoral Care, Social Workers, Volunteers and Nurses Aides.

Works to enhance the fullness of life for older adults living independently in the community. Provides services to seniors, which would help them remain in their homes, and continue to be a part of their community. Services include friendly visits, companionship assistance, transportation and shopping assistance, spiritual care, health education and counseling, network to community agencies, advocacy for quality care.

Provides emotional/spiritual support and advocacy to patients, families, and staff confronting crisis of illness. Support is ecumenical and holistic in nature, respecting individual faith traditions and concerns.

Breast Cancer Screening - Patient Navigation: These programs help women, regardless of insurance status, to overcome barriers to access and complete breast cancer screening at breast centers accredited through the National Accreditation Program for Breast Centers (NAPBC). New York State Breast and Prostate Cancer Peer Education: These programs offer education for women about the importance of regular breast cancer screening, and, for men, about the risks of prostate cancer. Provides children with access to high quality primary medical care. Currently accepting new patients and most insurances are accepted. Each practice is staffed by a team of highly qualified physicians and nurse practitioners. All physicians are faculty members at the University at Buffalo. Also participating in patient care are pediatric residents and medical students. Pediatrician available 24 hours for sick calls. Social worker and GYN services also available.
