Offers complete primary medical and dental care for entire family. Telehealth visits are now available in addition to face-to-face visits. Services include health maintenance, rapid HIV test (15 minutes), tuberculosis testing (by appointment), COVID-19 testing (via mobile health care unit), complete lab services, on-site pharmacy, GYN services, social work services, short-term counseling, pediatrics, full dental care, rapid lead testing for children, personalized education for chronic illnesses, and smoking cessation. Family Planning Benefits Program is a direct Medicaid public health insurance program for individuals and families who need family planning services, with no premiums or co-pays for services. Program is intended to increase access to confidential family planning services and to enable individuals of any age and sex to prevent or reduce the incidence of unintended pregnancies. ##REENTRY##

Grants life enriching wishes to eligible seniors 65 and older. Wishes have included traveling to see a loved one, attending a cultural or sporting event, participating in a favorite activity again, crossing off a bucket list item, taking art, swimming, or music lessons, or having a simpler need fulfilled. Seniors must meet income guidelines or permanently reside in a care facility. Also runs a HERO Box program to recognize the service of senior Veterans 65 and older. Offers charitable transportation options for those in need of assistance in getting transportation to receive medical treatment. Assistance is provided in the air with commercial airline tickets and volunteer pilots and on the ground with gas cards, bus and train tickets. All requests for rides are made online at http://patienttravel.org/.: Commercial Airline Transportation: administers charitable or deep-discount airline ticket program for patients and patient escorts on behalf of major airlines. Ambulatory patient travel exceeding 750 miles necessitates the use of airline resources. Special circumstances can make it advisable to use these programs for even shorter distances. Ground Transportation: ensures travel for low-income patients by providing gas cards and commercial bus or Amtrak tickets. Patients (with or without escort) must be ambulatory and require no medical monitoring or care en route. These trips are non-local medical care over 50 miles away and the typical trip does not exceed 300 miles. Volunteer Pilot Organizations: volunteer pilots donate their time and plane to transport patients to life-saving medical care. Make-A-Wish Central New York is a nonprofit, tax-exempt organization dedicated to granting life-changing wishes for children with critical illnesses throughout the central portion of New York State
Provides transitional housing for those re-entering the community from inpatient hospitalization and/or for those who have not benefited from traditional residential or supportive housing programs. Provides case management, information and referral, transportation, benefit advisement, on-call and after hours support, advocacy, and support groups via a multi-unit apartment building and the drop-in center. Offers short-term respite with goal of preventing crisis and hospital diversion.
