Eligibility

Families with a child or children under 21 who are Medicaid eligible and meet one of the following criteria: - Have 2 or more chronic health issues such as diabetes, obesity, allergies, asthma, or a hearing disorder - Have at least 1 of the following diagnoses: serious emotional disturbances, serious mental health issues, complex trauma, or HIV/AIDS Service Area: Erie County.
