Provides transportation for adults with Severe Persistent Mental Illness (SPMI) within Niagara County to day programs, medical or rehabilitation appointments, and services. Program operates in Niagara Falls, Lockport, and North Tonawanda areas. In addition to individual appointments, daily routes are established for transporting to and from CMI community support program sites.
Provides assistance in filling out forms, as well as information and referral. Offers portal to portal transportation for medical appointments and limited groceries. Also offers hot nutritious lunches, recreational activities, educational and health programs (including blood pressure, hearing, and cholesterol screenings), and organized trips (both day and overnight). Monthly bulletin available by request. Mayor's discount card available for all seniors 55 and older; must be used in the city only.
