Provides affordable housing for homeless Veterans in brand new, move-in ready apartments, with rental subsidies covering up to 100% of rent. Some residents may also qualify for utility allowance, depending on income. Robust case management services are provided to residents. Each apartment includes a bed, bedding, towels, kitchen accessories, cleaning supplies, personal hygiene items, and some furniture. ##Veteran##

Project Veterans' Independence is a Catholic Charities program partnership with the Department of Housing and Urban Development (HUD). Our goal is to place homeless veterans into permanent housing and help them live independently within their communities while offering counseling and access to support services. Supportive case management, peer-to-peer support groups and crisis intervention services are available to all clients.

JHA is a permanent supported housing program for homeless households containing at least one member with a disabling condition. One, two, and three bedroom units are available. On-site case management services help tenants access services and supports needed to stabilize housing and address barriers to future success.

Offers 68 scattered site, furnished apartments to chronically homeless individuals as part of a Housing First Model. The Housing First Model does not put conditions on housing; treatment and sobriety are not conditions, and income and ID are not required. Low to medium demand supportive case management is provided. The resident will be assisted with obtaining food, appropriate clothing, identification, benefits, medical services, and follow ups. Opportunities for work activities to develop skills and income potential are made available.
Mental health mobile outreach team providing assistance in obtaining housing, case management, mental health counseling, psychiatric evaluations, medication therapy, and placement into Shelter Plus Care housing units. Provides services to individuals with both substance abuse and mental health disorders (MICA). Operates supported housing units for homeless individuals with psychiatric disorders.

HATAS Shelter Plus Care Program is a supportive housing program that provides rental subsidies and case management for homeless people disabled by mental illness in Albany County. Consumers are linked to treatment and community services and are encouraged to reach their highest level of independence and self-sufficiency. Life skill building and support are provided.
Assists participants receiving Medicaid; and have one or more documented physical disability, and two or more chronic conditions (e.g., asthma, diabetes). All participants must fit in the category of being unstably housed/or homeless, and high utilizers of Medicaid. Referrals are to be made by Hospitals, Managed Care Organizations, Medical Respites, Performing Provider Systems, or a Skilled Nursing Facility. Assist individuals with the eligibility process, locating and subsidizing housing units; and obtaining services necessary to support successful living in the community. Community Transition Services (CTS) are available, on a case by case basis, to assist with security deposits, moving expenses, startup utility payments, and household furnishings/items.
Provides women with emergency, temporary, and permanent housing. Offers referrals for education, employment placement, medical care, chemical dependency issues, social services, and basic needs. Assists with temporary financial assistance for women and their children who are homeless due to domestic violence. Assistance may cover relocations fees, moving costs, security deposit, and 9 months of rental assistance.

Assist participants, who have Medicaid, are 55 and older or 18 and older with a documented chronic disability. OHS is intended to help high-needs Medicaid recipients that have lived in a nursing home for 120 days and are homeless. Housing Specialists assist people with enrollment in the Olmstead Housing Subsidy Program; serve as a liaison with landlords; conduct inspections of rental units to ensure health and safety; and hold monthly calls with all participants. Participants pay 30 percent of their monthly income toward rent for an apartment. If participants qualify; additional funding could be available for security deposits, moving expenses, and basic household furnishings.
