Program is available to those individuals who require more support, but no longer need a community residence. Staff members visit on an as needed basis and assist in medication monitoring and additional services. Must meet HUD criteria.

Serves Nassau County residents 18 and over with a primary psychiatric diagnosis. For certain locations, individuals must also meet HUD requirements for homelessness. Referral required. OASAS program-primary substance abuse diagnosis and subsequent hospitalization in addition to homelessness.
