Provides skilled nursing care, home health aides, personal care aides and homemaker services to the sick, frail, elderly or anyone in need of home health care and/or companion services in the five boroughs of New York City, Long Island and the County of Westchester. This includes, but is not limited to Hospice, EISEP clients, Mental Health and AIDS patients. Offers healthcare professionals and social providers in the areas of geriatrics and gerontology continuing education and training programs focusing on primary care, health promotion, disease prevention, multicultural aging and outreach services. Provides a Geriatric Resource Center with a wide range of resources along with a Speaker's Bureau. Innovations in the geriatric field and clinical updates are disseminated through interdisciplinary courses, team training, distance learning and workshops.

Offers a non-credit-bearing certificate program leading to eligibility for the licensure examination and PN licensure. This program includes classroom theory, theoretical knowledge, and clinical experiences to prepare students to take the NCLEX-PN for licensure as a Practical Nurse and the opportunity to pursue advanced college training. The Eastern Suffolk School of Practical Nursing offers the following programs each September: An 11-month, full-time day program which meets September through August, M-F from 8:00 a.m. to 2:30 p.m.; A 17-month, part-time evening program which meets September through June, M-Th from 6:00 p.m. to 9:30 p.m. AND the following September through March, M-F from 8:00 a.m. to 2:30 p.m. and A part-time day program is available to seniors enrolled in the high schools of the first supervisory district which meets September through June, M-F from 7:45 a.m. to 10:15 a.m. OR 12:00 noon to 2:30 p.m. at the Bixhorn Technical Center in Bellport, NY; AND will meet the following September through March, M-F from 8:00 a.m. to 2:30 p.m. for Level II post secondary at the Adult Education Center in Brentwood. Offers an exciting program that prepares Registered Nurses and Physician's Assistants for a career in the legal field as legal nurse consultants. This course provides the RN and PA with the fundamental skills necessary to advise law firms, health care providers, insurance companies and governmental agencies regarding medically related issues and to appear in court as an expert witness. The course presents legal concepts related to the health care industry, as well as the role a legal nurse consultant might play in litigation areas such as medical malpractice, toxic torts, products liability, wrongful death, criminal law, and workers' compensation. Offers courses in Dental Chairside Assistant; EKG Technician; Clinical Medical Assistant; Home Health Aide; Nurse Assistant; Phlebotomy Technician; Practical Nursing Program; Medical Office Biller/Coder ICD-10; Preparation For Practical Nursing Entrance Exam; Nurse Entrance Exam; Physical Therapy Aide; Certified Personal Trainer and Reiki 1,2,3. After completing the required amount of hours of training some courses above offer certification. All programs offered at various sites throughout Suffolk County in the day as well as the evening. Selection varies each semester.

Delivers planned and coordinated services to meet each patient's specific home health care needs including: registered nurses, home health aides, personal care aides, live-ins, and companions. Provides total case management including contacting the patient's physician, arranging for equipment needs and works closely with family to assure the best care for every patient.

Provides in-home skilled nursing care, rehabilitation therapies, medical social workers,home health aides, intravenous therapy, medical supplies/equipment, laboratory services and pharmacy supplies. Also offers Mom and Me Care to new mothers and newborns. Home care is provided by nursing staff from the maternity department.

Offers a variety of educational opportunities for professionals who work with youth, including conferences, workshops, trainings and networking with a focus on adolescent sexual health and adolescent development.

This service is for divorced, unmarried or separated families who are typically referred by the Family or Supreme Court. Based on court order the program can provide Therapeutic Supervised Visitation or Monitored Exchanges. Visitation provides children with a safe, secure and healing environment where they can have contact with their non-custodial parent. Typically, these are families with allegations and/or a history of mental illness, physical/emotional abuse, alcohol or chemical dependency, low parent skill or a risk of abduction, and where the court has determined that supervision is required. Exchanges provides a neutral drop-off and pick-up service that allows children to have safe transitions for scheduled access time. This is a service for families where safe child access to a parent is jeopardized because of existing orders of protection, domestic violence and/or high parental conflict.

Provides a 49-bed supervised living facility which includes personal care aides, housekeeping, case management, recreation, medication management, meals and nutrition education. Serves frail elderly who can no longer live on their own.
