Provides in-home relief of primary care giving responsibilities for the caregiver. The program services developmentally disabled, adults and children with OPWDD eligibility in Nassau and Suffolk counties living in their family home.

Provides education, evaluation and clinical services for children with disabilities ages 3-5. Clinical services include speech and language therapy, physical and occupational therapies, as well as psychological and social work services and parent support services such as groups, training and workshops.

Places trained providers in the home to provide rest and relief for the caregiver.

Provides support to individuals, 18 years of age and older with developmental disabilities transitioning from school, in developing skills for independent living and employment. Provides specialized information and referral to persons with physical disabilities who are seeking assistive technology devices to maintain or improve their functional capabilities. Provides referrals for assessments, and will discuss specific devices. Technology-Related Assistance for Individuals with Disabilities to choose, acquire and use assistive technology appropriate for their needs. Collaborative Project of the United Cerebral Palsy Association of Nassau County and United Cerebral Palsy Association of Suffolk County.

Provides mentally and developmentally disabled adults with independent living skills instruction and money management strategies. Offers health and wellness groups, relaxation techniques, speech and physical therapy. Provides assistance in community integration. Conducts needs assessment, treatment and discharge planning and medical referrals.

Conducts evaluations (at home or community school) for children birth through 3 years old, suspected of having developmental delays. Qualifying children may be eligible for services in speech, occupational and physical therapy and special instruction. Evaluation and services are free to Nassau County residents.

Offers a 10 week session to promote positive peer relationships among children with an autism spectrum disorder and/or a developmental disability. The sessions are comprised of role playing, emotional recognition, social stories and scripting. This is a private pay program and the fee is based upon the families yearly income. Provides service dogs to be matched with adults, children ages 18+ or veterans with a disabilities other than blindness; and facility dogs for professional therapists/health care workers to use in animal-assisted therapy programs within hospitals, nursing homes, rehab centers or other such institutions.

Provides acute, intense, short-term, inpatient psychiatric treatment to adolescents ages 12-17. Aims to achieve rapid stabilization to make possible ongoing treatment in a less restrictive setting. A variety of behavioral and emotional problems are treated including: anxiety disorders, Attention Deficit Hyperactivity Disorders and other disruptive behavior disorders, mood disorders including major depression, bipolar disorders and psychotic disorders. Helps people get answers to questions about supports and services for individuals with developmental disabilities in New York State. Also takes complaints of alleged misconduct, fraud, waste and abuse in the OPWDD service system. Requests for information will be referred to someone who will return the call. Representatives available in English and Spanish speaking or translation services available for other languages.

Provides opportunity for six teens with developmental disabilities to participate in existing Counselor In Training program at Great South Junior Travel Camp. Offers opportunity to assist counselors in providing recreation activities to children, and to participate in recreational activities with their peers. Three or six week sessions available. Provides Adult Day Habilitation to developmentally disabled individuals ages 21 and over who reside in Suffolk/Eastern Nassau with their families, or in agency-operated group homes.

Provides residential treatment services to children and adolescents who suffer serious emotional difficulties 5 to 17 years of age in five homes in Nassau and Suffolk Counties(Bethpage, Farmingdale, Islip, Manorville, and Plainview). Services include counseling, daily living skills, education/vocational support, socialization, family support groups, and family therapy.

Provides art classes including one-stroke painting, drawing workshops for beginners, drawing clinics, drawing workshops for developmentally disabled and beginner painting(oils and acrylics).

Provides residential services to developmentally disabled individuals who exhibit challenging behaviors. These homes are located on both the main campus in Smithtown and DDI's Huntington Campus. Children in residence attend Developmental Disabilities Institute Day School and then return to their residence where supervised recreation and daily living activities continue.

Offers diagnostic and treatment services to children with developmental problems and associated behavioral disorders, from birth through adolescence. Programs include: outpatient and inpatient pediatric consultation preschool and school-age learning diagnostic program neonatal developmental follow-up program Early Childhood Development Program which includes infant/toddler and preschool Early Intervention Program and Adoption Evaluation Center.

Offers touring musical reviews and productions to schools and community organizations. Includes drama workshops to senior organizations and pre-school curriculum based productions. All productions promote inclusion of both actors and audience members.

Provides site based respite services and Supplemental Day Habilitation which includes social and recreational opportunities both in-house and in the community to developmentally disabled adults over the age of 21 who reside in Nassau and Western Suffolk Counties.

Provides an opportunity for social interaction to further specific skill development relating to handicapped condition, and to encourage individuals to advocate on their own behalf. Provides referrals to respite and recreation programs. Support groups for siblings of the disabled. A mental health based program provides a range of psychotherapeutic service for individuals with highly specialized needs including the physically and developmentally disabled. Methodology includes individual, family, group counseling, advocacy, tutoring, socialization and skill training.

Provides adult respite care for developmentally disabled individuals, age 21 and over, who reside in Suffolk/Nassau counties with their families. Limited geographic area may be eligible for transportation periodically. Universal medical application and site visit appointment required.
