Provides habilitation services within a person's private home or apartment. Hours provided by staff vary based on a person's needs. The main goal of this program is to increase independence while providing support to the person and the family. People are encouraged to strive to live independently in the community with minimal staff support.

Helps people get answers to questions about supports and services for individuals with developmental disabilities in New York State. Also takes complaints of alleged misconduct, fraud, waste and abuse in the OPWDD service system. Requests for information will be referred to someone who will return the call. Provides an Early Intervention(EI) and Committee on Preschool Special Education(CPSE) related services for children ages 0-5 who have developmental delays in areas including: communication, physical and motor, cognitive, social and emotional, and adaptive skills. Performs evaluations in all areas including diagnosing autism spectrum disorder. Offers individual treatment plans including: speech and language therapy including PROMPT physical and occupational therapy, special education, feeding therapy, nutritional and parent counseling, sensory integration training, play therapy, applied behavioral analysis (ABA), and education of the visually and hearing impaired. Serves children with a diagnosis of autism or pervasive developmental disorder between the ages of 18 months and 6 years old. Provides intensive full day special education. Students receive individualized instruction throughout the day. Children have the opportunity to access both small and large group instruction, access to typical peers through peer modeling and community based inclusion.

Provides various levels of support to individuals who are living on their own or with their families. Included is in-home respite services, crisis support, family reimbursement, socialization and organized activity opportunities as well as site-based programming.

Provides mentally and developmentally disabled adults with independent living skills instruction and money management strategies. Offers health and wellness groups, relaxation techniques, speech and physical therapy. Provides assistance in community integration. Conducts needs assessment, treatment and discharge planning and medical referrals.

Provides service dogs to be matched with adults, children ages 18+ or veterans with a disabilities other than blindness; and facility dogs for professional therapists/health care workers to use in animal-assisted therapy programs within hospitals, nursing homes, rehab centers or other such institutions.

Infants and toddlers from birth to three years of age receive comprehensive evaluations in the family's home. If eligible by diagnosis or documented delay, services are provided in natural environments (home, community/day care facilities) through the Early Intervention Program as authorized by Suffolk County Bureau of Services for Children with Special Needs. Evaluation and services available include special instruction, speech, occupational, physical, vision therapies, social work services, and psychological services.

Serves children ages 5-21 with a diagnosis of autism, intellectual disability, or multiple disabilities. Teaches language and communication skills, self-help and activities of daily living, and behavior management. Other services include crisis intervention for families and parent education, counseling and support.

Offers fully accredited high school program for Deaf adolescents a work/study experience program which is community based a mainstream program with neighboring public schools and an alternative high school program.

Provides adult respite care for developmentally disabled individuals, age 21 and over, who reside in Suffolk/Nassau counties with their families. Limited geographic area may be eligible for transportation periodically. Universal medical application and site visit appointment required.

Provides comprehensive Medicaid case management for individuals, children and adults, who have an IQ of 70 or below. Provides home and community based waiver service. Individuals must be Medicaid eligible.

Monitors regulatory compliance by service providers within the district. Provides technical assistance and training to consumer agencies, maintains medical records provides program evaluations as well as statistical and analytical reports, and provides oversight and technical assistance to agencies that offer services to developmentally disabled individuals in Nassau and Suffolk counties. Provides an early intervention program for children birth-2.ll years. Program is center and home based for children with confirmed or suspected hearing loss and their parents. Offers a fully accredited elementary program recognized by New York State for deaf students. Also offers full after-school extended program including sports and computers. Provides Saturday recreation program for autistic children through 12 teaching them leisure skills and giving respite opportunity (at the same time) for their families

Provides audiological services to children birth to adults in communities across Long Island. Services are offered on a fee-for-service basis. Financial grants and sliding scale are available.
