Provides care management to individuals with a serious mental illness, HIV/AIDS, or two or more chronic health conditions and significant behavioral, medical, and/or social risk factors, and who are in need of assistance linking to services to stabilize and improve their overall health functioning. Program services include outreach and engagement, comprehensive care management with assessment and care planning, care coordination and health promotion, comprehensive transitional care, client and family support, referral to community and social support services, and use of health information technology to link to services.

Provides various services for the senior residents of Steuben County including, but not limited to, information and referral, nutrition programs, caregiver support services, health insurance information and counseling including long term care information, advocacy services, and transportation help. Offers various workshops and outreach programs.

Provides an opportunity for social interaction to further specific skill development relating to handicapped condition, and to encourage individuals to advocate on their own behalf. Provides referrals to respite and recreation programs. Support groups for siblings of the disabled. A mental health based program provides a range of psychotherapeutic service for individuals with highly specialized needs including the physically and developmentally disabled. Methodology includes individual, family, group counseling, advocacy, tutoring, socialization and skill training.

Offers Home Health Aids )HHA;S) Personal Care Aides (PCA;S), and companion care services throughout Nassau County. Services are provided on an hourly basis through private pay, long term care insurance, Office of the Aging, Medicaid, Workers Compensation, Managed Long Term care (MLTC), and Mainstream Managed Care (MCO) programs.

Promotes the acceptance of non-smoking as the social norm. Strives to meet the goals of prevention, cessation, and protection by working through media advocacy, policy initiatives, and existing tobacco, education and control programs. Also provides newsletter, cessation brochures in English and in Spanish, and a cessation poster.

Matches preschool age children with disabilities with typically developing children in their communities for one on one play dates.

Provides a meeting location and a mutual support environment for men and women helping each other to stop smoking and live nicotine-free lives. Each meeting location provides a 12 step style of commitment based on the philosophy of Alcoholics Anonymous. Meeting locations may be in Suffolk, Nassau, Queens, Brooklyn, and Manhattan on specific days and at varied times. There are currently no meetings in Staten Island and the Bronx. Provides occupational, physical and speech therapy, cognitive therapy, oral motor therapy evaluations, and early intervention services for children ages birth to 5 years of age with developmental delays, head injuries and other conditions that may impact developments. Call for more information.

Offers support to Nassau or Suffolk County residents recovering from substance abuse transitioning from inpatient treatment back home or into a sober living environment. Assists individuals in recognizing relapse triggers, addressing thoughts, beliefs and feelings and establishing a local support system. Develops an individualized relapse prevention plan.

Protect Allergic Kids (PAK) is a nonprofit arganization and support group that provides a wealth of information on food allergies, anaphylaxis and allergic diseases including Eosinophilic disorders and Celiacs. The PAK program is available to parents, caregivers, teachers and anyone who knows a child affected by these medical conditions. Topics will be discussed including: how to recognize an allergic reaction, safe practices and support at school for food-allergic children, explanation of various food-allergic diseases and resources such as allergen-free foods and books. Offers Speakers Bureau for Schools, libraries, Daycare Centers, etc.

Program assists seniors 60 years and over to remain in their own homes after retirement. Services offered: healthcare monitoring and screenings, connections to benefits and services, home maintenance assistance and minor repairs, counseling, case management and service coordination, and nutrition assistance, as well as social and recreational opportunities.

The Residential Repair Program exists for Town of Islip senior residents who are no longer able to perform small repairs in their home. Any materials needed must be supplied by the senior. Labor is supplied by Town of Islip residential repair workers. Although there is no charge for labor, contributions are appreciated and help maintain the program.

Provides an 18-bed inpatient unit as well as ambulatory and satellite programs tailored to the unique medical and developmental needs of teenagers and young adults. Services include: General Health Care, Eating Disorders Center, Weight Management Center, Atherosclerosis Prevention Center, Adolescent Gynecology, Deaf Health Services, Chronic Fatigue Syndrome Services, Behavioral Health Services, HIV Prevention Services and School Based Health Services.

Offers multidisciplinary treatment for patients with epilepsy or related disorders, including monitoring for surgical candidates, pharmacological management, experimental drug trials and psychological and social support services.

An assisted living community that specializes in senior living and offering independent living, assisted living, and memory care.

Serves boys, ages 10-17, who suffer serious emotional disturbance. Offers a highly structured, treatment oriented, cost effective alternative to placement in a psychiatric hospital. Located in McKeown House on the Syosset campus.

Provides benefits advisement, peer counseling, housing referral,advocacy, independent living skills, transportation, equipment loans, consumer directed personal assistance program (CDPAP), Hispanic outreach, and aging in place services to people with disabilities and the elderly.

Operates 24 hours a day, seven days a week providing private and semi-private rooms, meals, assistance with medication management, visiting physicians, laundry services, and housekeeping.

Serves children with a diagnosis of autism or pervasive developmental disorder between the ages of 18 months and 6 years old. Provides intensive full day special education. Students receive individualized instruction throughout the day. Children have the opportunity to access both small and large group instruction, access to typical peers through peer modeling and community based inclusion.

Provides community based habilitation to developmentally disabled individuals, ages 21 and over who reside in Suffolk/Eastern Nassau with their families. This program operates in and out of the individual's home, and can provide support in the morning, afternoon, evening or weekend.

Provides resources such as a lending library on mental health issues, computer and internet services, recovery, consumer issues in book, video and audio cassette to both individuals with mental illness and mental health care providers. Resources support recovery based models of rehabilitation. Coordinates the Long Island Committee on Sexual Abuse.

Provides and opportunity for seniors to serve as companions to developmentally disabled adults ages 21 and older. Offers a small stipend for low income seniors who act as role models and advocates Seniors must have a medical release, be free from critical illness, have a tuberculin test and must be low income.

Serves children ages 5-21 with a diagnosis of autism, intellectual disability, or multiple disabilities. Teaches language and communication skills, self-help and activities of daily living, and behavior management. Other services include crisis intervention for families and parent education, counseling and support.

Provides art classes including one-stroke painting, drawing workshops for beginners, drawing clinics, drawing workshops for developmentally disabled and beginner painting(oils and acrylics).

Offers a center based program for 2-5 year olds with delays in speech/language/cognitive learning areas with or without presence of hearing loss. One full day session is offered from 9:20am to 2:40pm. An integrated class comprised of both disabled and non-disabled students is also offered.
