Provides care management to individuals with a serious mental illness, HIV/AIDS, or two or more chronic health conditions and significant behavioral, medical, and/or social risk factors, and who are in need of assistance linking to services to stabilize and improve their overall health functioning. Program services include outreach and engagement, comprehensive care management with assessment and care planning, care coordination and health promotion, comprehensive transitional care, client and family support, referral to community and social support services, and use of health information technology to link to services.

Provides a meeting location and a mutual support environment for men and women helping each other to stop smoking and live nicotine-free lives. Each meeting location provides a 12 step style of commitment based on the philosophy of Alcoholics Anonymous. Meeting locations may be in Suffolk, Nassau, Queens, Brooklyn, and Manhattan on specific days and at varied times. There are currently no meetings in Staten Island and the Bronx. Protect Allergic Kids (PAK) is a nonprofit arganization and support group that provides a wealth of information on food allergies, anaphylaxis and allergic diseases including Eosinophilic disorders and Celiacs. The PAK program is available to parents, caregivers, teachers and anyone who knows a child affected by these medical conditions. Topics will be discussed including: how to recognize an allergic reaction, safe practices and support at school for food-allergic children, explanation of various food-allergic diseases and resources such as allergen-free foods and books. The Residential Repair Program exists for Town of Islip senior residents who are no longer able to perform small repairs in their home. Any materials needed must be supplied by the senior. Labor is supplied by Town of Islip residential repair workers. Although there is no charge for labor, contributions are appreciated and help maintain the program.
