The purpose of this program is to identify people in nursing homes who wish to transition back into the community, explain programs and services available to them, and make the appropriate referrals to begin the process.After their transition, participants are offered Person Centered Coaching for 6-8 skill building sessions to help identify strengths, preferences, needs, and desired outcomes. Open Doors also works closely with the Olmstead Housing Subsidy (OHS) Program, which provides housing assistance, furniture, and household goods to those who qualify.
