Putnam NY Connects promotes personal responsibilities to empower and create opportunities for consumers and their caregivers of all ages, allowing them to make informed choices about their Long Term Care needs that balance cost, access, and quality. Our aim is to help alleviate the frustrations of trying to navigate through a complex and fragmented long term care system.

Offers telephone crisis intervention for victims of domestic violence or rape/sexual assault. Trained staff and volunteers link callers to necessary resources and referrals while providing supportive listening.

Provides specialized information and referral to persons with physical disabilities who are seeking assistive technology devices to maintain or improve their functional capabilities. Provides referrals for assessments, and will discuss specific devices. Technology-Related Assistance for Individuals with Disabilities to choose, acquire and use assistive technology appropriate for their needs. Collaborative Project of the United Cerebral Palsy Association of Nassau County and United Cerebral Palsy Association of Suffolk County.

Provides services under the Department of Health Traumatic Brain Injury Waiver Program. This program assists individuals obtain in and out of home supports to maintain their optimal level of independence. Services provided include: service coordination, structured day program, community integration counseling and behavioral supports for individuals diagnosed with traumatic brain injury

Serves as a point of entry into the long term services for older adults and people of all ages with disabilities. Some of these services include: personal care services (preparing meals, housekeeping, bathing, transportation, home safety and accessibility and household finances), education and counseling, behavioral health/development supports and services, health and wellness, insurance information and Options counseling.

Offers a specialized telephone and online service that provides information about mental health resources and referrals to community members, professionals, consumers and agencies. Professional standards of confidentiality are followed for every caller.

Provides access to individuals and families who are new to this country through a network of education, advocacy and referral services to language skills, business planning, loan information, community engagement, and more.

Registers motor vehicles, motorboats, snowmobiles, and all-terrain vehicles. Licenses drivers, dealers, repair shops, inspection stations and driving schools, approves driver training programs and establishes uniformity in traffic regulations. Provides photo non-driver identification cards for all ages.

* Provides senior individuals with meals, socialization, activities, and other supports to assist participants in remaining independent and staying in their homes.

* Assists farmers, ranchers, and other agricultural workers with disabilities by offering the following services: On-site assessments of the agricultural worksite or home to identify barriers to completing tasks Recommendations for assistive technology (equipment/tools/devices), modifications that can make it easier to perform work, and/or other possible solution to overcoming disability-related limitations Access to information on a variety of topics related to disability and agriculture Provides education/training opportunities through workshops, conferences, seminars, and on-line programs Referrals to other local services for potential assistance (financial, rehabilitative, education, etc.) specific to the clients' needs Arranges for peer support opportunities that connect customers with others who have successfully accommodated their disabilities * Direct funding or equipment is not provided by the AgrAbility Project, instead the project works with state Vocational Rehabilitation departments and other funding sources.

* Provides support and information through a confidential crisis hotline for pregnant women who are hiding their pregnancy and need assistance with alternatives to reduce the rate of infant abandonment. * Coordinates a plan with woman who feel helpless and alone during an unplanned/unwanted pregnancy, many of whom are close to their due date and have no plans for once the baby is born. * Connects women with emotional support, baby supplies, parenting classes, shelter, medical care, employment, adoption assistance and other possible resources in the community. * Helps women consider using the safe surrender law.

* Refers families and concerned individuals to parenting education and support programs;Services for parents under stress.Legal services.Housing and emergency goods.Counseling.Anger management. Victim's services.Assistance navigating the family court and child welfare systems.

The Developmental Disabilities Services Division participates in planning, development and coordination of services to support people of all ages with developmental disabilities and their families. The division also provides advocacy, mediation, outreach, education and training for individuals with developmental disabilities, families, providers, educators, and the Orange County Community.
