Provides home and community based comprehensive care management, care coordination and health promotion, comprehensive transitional care from inpatient to other settings, including appropriate follow up, individual and family support, referral to community and social support services.

Offers a library of information and referral materials through the loaner library that can offer a wide variety of educational information on mental health issues, and other related topics. Also maintains a lists of psychologists or psychiatrists in the area if anyone needs a referral.

Offers an eight bed 24/7 supervised group home for adolescent girls and boys (ages 12-18), diagnosed as seriously and emotionally disturbed. Provides a therapeutic living environment to promote youth's highest level of social and emotional functioning. Our goal is for the youth and families alike to recover, reunite and work on transitioning the youth back into their homes and communities. Average length of stay is between 12-18 months.

Provides short-term, home based services to families with children who are in or were recently released from in-patient psychiatric facilities. Helps to prevent rehospitalization, enhance family functioning, improve child/parent relationships, and promote self sufficiency. Call for more information.

MHA of Westchester serves children, adolescents, adults, and families through individual, family, and group therapy and medication management. Our individualized care is integrated, flexible, and person centered.

Supported housing is permanent housing. The supported housing program has sites throughout Brookhaven and Riverhead consisting of 2 and 3 bedroom houses within 1/2 mile from shopping, banking and public transportation. Provides case management services with the housing. Also provides the security of permanent housing, the flexibility of independent housing and the support of OMH and HUD funded housing in one program.

Provides telephone calls to community members who are homebound or physically isolated who are suffering from psychological or emotional problems or who require a support system. Persons called can be self-referred or referred by physician, psychiatrist, mental health center, case manager, health care provider, or hotline counselor. Also provides therapist referrals and a referral service.

Provides affordable, low cost, high-quality psychotherapy services for individuals in our community. The following mental health services are offered: Individual therapy for children, adolescents, and adults; Group therapy for all ages; Couples therapy; Family therapy and Workshops and staff development training. The Mental Health and Wellness Center offers a variety of treatment options and support for a wide range of mental health issues and concerns, including: Depression; Anxiety; Body Image/Eating Disorders; Trauma; Personal Growth and Development; Family Crisis; Gender and Sexual Identity; Grief and Loss; Mental Health and Wellness and Short-Term Solution Focused Psychotherapy. Graduate students from the CMHC Program provide counseling services under the supervision of licensed clinicians and mental health professionals. Day and evening appointments are available. Provides staff-supportive phone lines to offer around-the-clock companionship and peer support to persons in distress or in need of someone to listen.

Provides learning experiences where individuals can develop strengths and skills that contribute to an individualâs employability in the community. Services include teaching concepts such as compliance, attendance, task completion, problem solving, safety, wellness while at work, socialization at work, communication, accepting supervision, personal appearance and appropriate work attire; goal setting, work ethics, and work tolerance.

Provides support and structure for individuals experiencing behavioral/emotional challenges. Clinical services provided for 3 to 30 consecutive days and assists with transition back to usual living environment. Referral required. Program run in collaboration with SUNY Stony Brook-Department of Psychiatry/Division of Developmental Disabilities.

Coordinate workshops, events and other formats of public awareness and prevention of suicide issues. Helps to educate people about suicide and the warning signs.

Provides a comprehensive, 3-part recovery oriented program for individuals with severe and persistent mental illness. Goals for individuals in the program are to: improve functioning, reduce inpatient utilization, reduce emergency services, reduce contact with the criminal justice system, increase employment, attain higher levels of education, and secure preferred housing.

Provides a variety of transitional housing services to individuals with a primary mental illness people may also have substance use issues.

An outpatient clinic for adults who are experiencing depression, anxiety, family conflicts, emotional or psychiatric illness. The program is offered by Psychiatrists, Social Workers, Clinical Nurse Specialists, and Nurse Practitioner. Specific treatment services include the following: Biopsychosocial Assessment and treatment planning Psychiatric evaluation Community consulation and education Verbal Therapy-individual, groups, couples, and family Psychopharmacotherapy Medication Management Crisis Management Health screening and referral Rehabilitation readiness Case Management.

Provides information and referrals to the various mental health services and resources in the county, including access to numerous videos, written material, and research on mental health topics.

Provides housing and psychiatric rehabilitative services to adult residents of Nassau County who have chronic mental illness. Call for more information.

Provides family support services to help families care for their children who have a serious emotional disturbance (SED) diagnosis or who are at risk of being diagnosed. Services include: individual meetings, support groups, workshops and family engagement activities.

Informs and assist members of the armed forces, veterans, their dependents and survivors in relation to (1) matters pertaining to education, training and retraining services and facilities, (2) health, medical and rehabilitation services and facilities, (3) provisions of federal, state and local laws and regulations affording special rights and privileges to members of the armed forces, veterans and their families, (4) employment and re-employment services, and other matters of similar related or appropriate nature. Peer facilitated psychosocial club for seriously and persistently mentally ill adults focused on socialization and empowerment. Members are invited to drop in for any of the gatherings. Offerings include but are not limited to: participation in community events, games, videos and crafts in a relaxed atmosphere designed to encourage interaction and reduce isolation. Transportation assistance is available when possible.

Assists in questions on mental health issues including: Symptoms of mental health conditions, Treatment options, Local support groups and services, Education programs, Helping family members get treatment, Programs to help find jobs, Legal issues (the NAMI Legal Resource Service can connect individuals with attorneys in their area but does not have the resources to provide individual representation). NOTE: This helpline does not provide counseling or therapy, specific recommendations for things like treatment or do individual casework, legal representations or other individual advocacy. Crisis calls, such as suicide ideation calls, are transferred to a national crisis line to provide further assistance.

Provides services for Suffolk County residents age 18 plus who are in recovery from mental illness. Call for details.

Volunteers needed as telephone crisis counselors for agency's 24-hour hotline and the support line which provides telephone visits to homebound individuals. Training in crisis intervention communication skills provided, consisting of 24 hours classroom hours and 24 hours hands on training in conjunction with an experienced hotline counselor. Shifts are available 24 hours per day, 7 days per week. Request commitment of one 4 hour shift per week for a minimum of 6 months. College-age and adults.

Offers outpatient programs and a wide range of services by mental health professionals. Our staff includes psychiatrists, psychologists, licensed clinical social workers and activities therapists. Patients can join individual, group and family psychotherapy, and we offer specialized services for depression, anxiety and childhood emotional disorders. Specific programs include: Adult Psychotherapy Program for individual or group therapy; Family and Children's Program offering individual, group and family psychotherapy for children and their families and Options for more intensive care at South Nassau: Partial Hospitalization Program, five days weekly; Intensive Outpatient Program, three days weekly and Inpatient Behavioral Health Unit.
