COMMUNITY RESIDENCE PROGRAM - DIX HILLS | WELLLIFE - DIX HILLS
Offers an eight bed 24/7 supervised group home for adolescent girls and boys (ages 12-18), diagnosed as seriously and emotionally disturbed. Provides a therapeutic living environment to promote youth's highest level of social and emotional functioning. Our goal is for the youth and families alike to recover, reunite and work on transitioning the youth back into their homes and communities. Average length of stay is between 12-18 months.

A referral is required through the Suffolk County Single Point of Entry, (631)853-2995. Youth must have a birth certificate and social security card. Parental consent is required.
