* Provides home health aides, licensed nurse practitioners, registered nurses, attendants, and personal care aides to work and oversee medical treatment in the home. Some of the services provided are: Medication reminders Grooming and oral hygiene Bathing and dressing Mobility issues Meal preparation Laundry Housekeeping Errands Companionship* Provides a full range of services under the TBI/NHTD Waiver that includes the following: Home and Community Support Staff, Community Transitional Services, and Moving Assistance and Transportation. * Offers non-medical transportation services.* Provides care management through Health Homes of Upstate NY (HHUNY) and Children's Health Homes of Upstate NY (CHHUNY).* Offers tuition free certification for Personal Care and Home Health Aides.

Provides a variety of services in home and community based settings to assist with improving the quality of life for people with disabilities and their families via OPWDD and OMH. Supports include respite, community habilitation, recreation, after school programming, summer programming, broker services, early intervention services, children's mental health services, intensive parent training, children's health homes, and employment services.

Provides caregivers assistance with accessing resources, individual counseling, resource library, respite services, Alzheimer's support groups, community educational programs and guest speakers.

The Office for the Aging offers many services for the senior residents of Steuben County including, but not limited to, information and referral, nutrition through congregate meals, advocacy, support groups, transportation services, caregiver services, case management, and the Bone Builders and Senior Meal Programs. Bone Builders and the Senior Dinner Clubs are overseen by Pro Action of Steuben-Yates.

Offers information, referral, and assistance to obtain needed services. Provides counseling and consultation on caregiving issues and long term care planning. Offers support programs such as In Home Respite Care, Personal Emergency Response System, and Adult Day Services.

Provides temporary relief during the day or late afternoon/early evening, including weekends. In home respite workers will help families experience success by providing skills supervision of a child and siblings, if needed.

Nursing care and rehabilitation facility provides long and short term care for those in need of 24 hour nursing care due to chronic illness, injury or advancing age. Physical, occupational and speech therapies are provided, as well as respite and hospice care.

180-unit nursing home certified respite care available. Nursing homes or residential health care facilities provide 24-hour nursing care and supervision outside of a hospital Nursing homes are licensed and regulated by the New York State Department of Health.

Provides site based respite services and Supplemental Day Habilitation which includes social and recreational opportunities both in-house and in the community to developmentally disabled adults over the age of 21 who reside in Nassau and Western Suffolk Counties.

49-unit supervised home for older adults in addition to an enriched living program. Respite care available. Serves people who are memory impaired.This facility is licensed as an adult home. Adult homes provide long-term residential care with 24-hour supervision and a full range of services such as meals, housekeeping, some personal care and social activities. Adult homes are licensed and regulated by the New York State Health DepartmentEnriched housing programs provide seniors with long-term residential care in apartment settings resembling independent living units. Additional services are available such as meals, housekeeping, social activities and some personal care.

Provides voucher reimbursement for at home respite to families of developmentally disabled children, age 3 to young adult, residing in Suffolk County. Caregivers provide care to the disabled individual allowing family members time off from their usual caregiver responsibilities.

Our Intake Department facilitates access to short-term Planned Respite services to help caregivers continue to provide long-term support for a child with developmental, mental health, or health care issues. A direct care service provides activities and skills development to a child while maintaining the child's health and safety.

Operates residences and apartments located in neighborhoods throughout Rockland County to help individuals with developmental disabilities to develop critical life skills to increase independence, enjoy community life as well as enjoy social and recreational opportunities. Preauthorization for Residential Services is done through OPWDD.

Provides individualized, strength-based approach to serve children with mental health and substance use needs. Provides tools to youth and their families to support the ability to make informed decisions about care. Services include care coordination, respite, family support, skill building, intensive in-home services, and crisis response.

* Assists individuals with developmental disabilities: In-Home Respite - gives caregivers and those served an opportunity to spend some time apart Day Habilitation - includes training and assistance in personal care, basic safety skills, social and recreational activities, relationship building Community Habilitation - community living setting with skill-building including money management, menu planning, and food preparation, chore completion, and recreation Individualized Residential Alternatives (IRAs) - provides a group living environment with services tailored to each resident's individual needs

Provides in-home personal care Levels I and II, case management for non-Medicaid clients. Provides assistance with activities of daily living such as bathing, dressing, food preparation and household chores.
