Provides family support services to help families care for their children who have a serious emotional disturbance (SED) diagnosis or who are at risk of being diagnosed. Services include: individual meetings, support groups, workshops and family engagement activities.

Provides a broad-based system of community services to assist individuals with disabilities solve problems faced in their daily lives. Services include: Case Management, respite and services in the home are offered. Group homes for developmentally disabled, family care, and medicaid coordinators available.

Helps improve the quality of life for individuals living with Alzheimer's Disease and related memory disorders and their caregivers. Also offers a unique social model day program, in home respite, support groups, Brainfitness Workshops, Whimsical Wednesdays, and educational workshops.

Provides site based respite services and Supplemental Day Habilitation which includes social and recreational opportunities both in-house and in the community to developmentally disabled adults over the age of 21 who reside in Nassau and Western Suffolk Counties.

Offers a variety of respite services to suit personal needs. Respite is temporary care away from home, and is crucial for many families caring for loved ones with developmental disabilities.

Hearts and Hands is a volunteer based non-profit organization that provides free escorted transportation, companionship, and in-home supports for older adults in communities throughout Erie and Eastern Niagara Counties. Our care recipients are often individuals who stopped driving due to age-related medical conditions or for financial reasons. Recipients may also need assistance at home with light yard work and home maintenance, or companionship to renew and refresh their caregiver. Our elderly neighbors are often lonely, isolated, and in need of friendly human interaction. In response to a recent community assessment and a national assessment from AARP, Hearts and Hands has designed our program to address social isolation through all three avenues of service: transportation, household supports, and companionship. Hearts and Hands volunteers may be the only people our care receivers see during a week, and their company is, in and of itself, a bright spot in daily life. Hearts and Hands is a best-efforts program. Services are provided by volunteers who give their time as they have it available. Assists families with an intellectually and/or developmentally disabled individual who reside at home.

Offers temporary crisis support and assistance with payment for a respite relief worker to assist in caring for a child during a challenging time.

Provides in-home relief of primary care giving responsibilities for the caregiver. The program services developmentally disabled, adults and children with OPWDD eligibility in Nassau and Suffolk counties living in their family home.

Offers hourly respite services for families of children/adults with developmental disabilities who need to take some time off from caregiving duties.

Provides in-home respite care and offers relief to primary caregivers of people who are frail elderly or developmentally disabled. In home meal preparation, in-home companionship. Offers escort for senior family member/friend to a doctor's visit, on a shopping trip, run errands, visit relatives, go to a movie and dine out. Escorts are carefully screened and trained and are matched to meet the individual's needs.

Operates residences and apartments located in neighborhoods throughout Rockland County to help individuals with developmental disabilities to develop critical life skills to increase independence, enjoy community life as well as enjoy social and recreational opportunities. Preauthorization for Residential Services is done through OPWDD.

Provides in-home respite care services, information and referral to caregivers resources in the community, including caregiver counseling. Also offers a monthly support group.

Planned Respite provides pre-planned short-term relief for a child’s/ adult’s primary caregiver and supports the child’s/adult’s developmental, behavioral, and health care needs.
