Unit is also responsible for administering the Long Term Home Health Care Program, designed to meet the needs of the chronically ill and disabled person in their home.
Offers several home care and long term care programs, depending on eligibility and care needs, to individuals who require assistance at home due to age, illness, post hospitalization, or disability. NOTE: The location 18 Sussex Street in Port Jervis is the Employment Training Center. However, they will accept documents to be submitted there for Department of Social Services.

Volunteers rake leaves, complete other outdoor yardwork, wash windows, inside and out (first floor only), do interior cleaning, vacuuming, etc.

An alternative to "traditional home care/personal care", CDPA allows eligible individuals to hire and direct their own personal care workers (including most family members, a neighbor or friend), to assist with bathing, dressing, eating, and other daily tasks. Eligibility is determined by the appropriate county entity. CDPA believes there's no place like home.

Serves residents of Richford and Berkshire, NY only. All services are free and confidential.
