Offers peer advocates to assist individuals with mental illness or developmental disabilities, to establish goals, and increase their independence. Peer counseling includes vocational assistance, budgeting, self-advocacy, interpersonal and independent skills training. Also provides benefits advisement, to help in navigating public assistance and Social Security, along with assistance with the Medicaid Buy-in program, teaching consumers to fill out forms, or helping them to understand work incentive programs.

Putnam NY Connects promotes personal responsibilities to empower and create opportunities for consumers and their caregivers of all ages, allowing them to make informed choices about their Long Term Care needs that balance cost, access, and quality. Our aim is to help alleviate the frustrations of trying to navigate through a complex and fragmented long term care system.

Provides support services to women who are affected by the incarceration of their spouse or significant other, and are caring for their children. These women can access peer support, receive counseling, and attend a unique weekend workshop, Loving Out Loud, which focuses on relationships with incarcerated men and solutions-based counseling.

NY Connects provides comprehensive, free information and assistance on long term supports and services to individuals of all ages, incomes, disabilities as well as family, caregivers, professionals and friends and neighbors.

A monthly networking support group for men who have had any type of cancer, at any a stage, including those who are post treatment. The group is informational and supportive and facilitated by a licensed mental health professional.

Offers pediatric support groups including: Hope for Hearts, a support group for parents and siblings of children with congenital heart defects Our Little Heroes/Oncology Parent Support Network, offering mutual support and guidance for families of children with cancer. Links person with breast cancer with breast cancer survivor for information, support, encouragement. Link may be made at time of diagnosis or at any time during person's experience with breast cancer.

Offers the following services for women who have been diagnosed with breast cancer: laboratory, pharmacy, physical therapy, social services, radiography, MRI, Magnetic Resonance Imaging, CT Scanning, Computerized Tomography, nuclear medicine and ultrasound in addition to a full-range of psycho-social and nutrition services. The Tumor Registry has a database of over 6,700 cases established since January 1983. The center also offers support groups for women newly diagnosed with breast cancer. Other support groups are for their caregivers, spouses and significant others. In addition there is an extensive library of books, pamphlets and videotapes on risk factors and prevention.

Provides care for children with chronic asthma and allergy problems. Services include: Consultations with Pediatric Pulmonologists and Allergist Immunologists skilled nursing care history and physical exam pulmonary function studies allergy testing laboratory services pharmacy services and an asthma support group.

Provides support and information to families with special needs children. Also matches parents, by request, with a trained support parent who has a child with a similar disability, chronic illness, or health care concern to help with parenting issues. This helps to build a support network and connect to tools and resources in the community. Also offers speakers to educate health care professionals, teachers, and others who care for children with special needs.

Offers a program to support, guide and comfort parents of newly diagnosed children with arthritis and those facing difficulties, and links them with other parents who have been through similar situations.

A twelve-step program for women with problems related to food including, but not limited to, compulsive overeaters, those with binge eating disorder, bulimics and anorexics. Meets every Thursday in Conference rooms 4 and 5. For information text Terri at (631) 241-2403.
