* Provides Care Management at Villa of Hope that supports adults in the community with goal setting, and connection to providers and community resources to create lasting stability.

* Provides coordination between all types of care a participant may need: behavioral health, medical and social services.

* Provides parents who have given birth and pregnant people the support they need to be physically healthy, emotionally healthy, and self-sufficient. Services include: On-site mental health counseling Pregnancy education, financial, parenting, postpartum, and life skills classes Job training Community Health Workers Care management

* Offers Non Medicaid Care Management services, Assertive Community Treatment Services and Mental Health Residential Services.

* Provides Children's Care Management for youth with Medicaid, and Care Coordination services for youth without Medicaid. Supports youth with a mental health diagnosis or history of trauma in meeting their goals and getting connected to the resources they need.

* Provides support to people with disabilities and their families in Seneca and Cayuga counties. Benefit management assistance. Legal and educational advocacy. Crisis assistance. Individual habilitation services. Family reimbursement. Parent support. Guardianship information. Respite assistance. Summer camp and recreation for adults and children. Transportation to doctor's appointments and planned shopping can be provided.

* Works to ensure all those involved in individual care are working together and sharing information in supporting a person's goal. Coordinate, advocate, navigate, and link individuals to resources.

* Provides service planning and assistance in securing access to services to help individuals in their recovery.* Assists adults, young adults, and youth that are experiencing chronic medical, mental health and/or substance use disorders conditions with care coordination and related community-based services.

* Offers individuals with disabilities and seniors who are eligible for nursing home placement to receive services in their home and community. * Provides home and community support for individuals with disabilities, traumatic brain injuries, and seniors.

* Matches individuals with a care manager to develop a plan to meet medical needs, including: Care management Home Health Aides Medication Management Medical Equipment Physical therapy Speech therapy Respiratory therapy Occupational therapy and more

* Assists with housing, employment and other coordinated care services for individuals with multiple health-related needs.

* Offers a broad array of services which include: Treatment Residential: Community residences. Supportive apartment programs. Single room occupancy (SRO). Crisis Respite. Supported housing. Transitional housing. Children and Youth Care management. Adult Health home care management. Forensic case management. Outreach and non-medicaid case management. Recovery support services for individuals recovering from both mental health and substance use disorders.

* Offers nursing support, advocacy and assistance coordinating doctor appointments, and help with understanding medical care plans and documents.* Accompanies patients to medical appointments, coordinates transportation for appointments and medical treatments.* Provides assistance with filling prescriptions and ensure linkages are made with other supportive community-based services.

* Authorization of services to individuals with traumatic brain injury or stroke. Waiver allows payment and budgeting for services. Participant may be elegible for rent subsidies and husing supports and limited one-time payment for furtniture and household supplies.* Management of Medicaid insurance money.* Linkage to services.

* Aims to reduce the incidence of shootings, killings, and other acts of violence using a public health approach.
