DCOFA CONTINUING CARE | DUTCHESS COUNTY OFFICE FOR THE AGING

Office for the Aging Continuing Care provides access to anyone, regardless of age and payment source, to long term care services. Case Managers and Public Health Nurses can assess the clients in their home, the nursing home or in the hospital, establish a realistic plan of care, and provide advice on available funding sources. They may also make suggestions regarding other alternatives such as Adult Day Care, Adult Homes, Assisted Living, Foster Care, Home Health Care, Respite Services, and Nursing Home placement. Office for the Aging / Continuing Care can help arrange for the provision of services, which may include in-home care such as homemaking and personal care, help with paperwork, etc.

OPWDD Supports and Services (Developmental Disabilities) | AIM Independent Living Center - Chemung County Outreach Office

Provides supports and services for people with developmental disabilities under contract with the NYS Office for People with Developmental Disabilities (OPWDD), including Service Access, Brokerage, Self-Directed Services, Community Habilitation, and Family Support Services. Family Support Services include monthly youth groups, social skills classes, and summer day camps.

LINK-AGE PROJECT | FAMILY AND CHILDREN'S ASSOCIATION

Case management for seniors with behavioral health needs. A team including an RN, case counselor and peer will provide confidential, comprehensive in-home assessment, develop a care plan and provide intensive short-term case management for seniors 55+ struggling with behavioral health, substance use or cognitive impairments that interfere with accessing community services and resources. Barriers to accessing services will be addressed.
