Office for the Aging Continuing Care provides access to anyone, regardless of age and payment source, to long term care services. Case Managers and Public Health Nurses can assess the clients in their home, the nursing home or in the hospital, establish a realistic plan of care, and provide advice on available funding sources. They may also make suggestions regarding other alternatives such as Adult Day Care, Adult Homes, Assisted Living, Foster Care, Home Health Care, Respite Services, and Nursing Home placement. Office for the Aging / Continuing Care can help arrange for the provision of services, which may include in-home care such as homemaking and personal care, help with paperwork, etc.

Assists veterans with long-term care needs to stay in their own homes. Design to help qualifying Veterans to hire, train, and supervise care attendants. Personal Care Attendants help with the Veteran’s daily needs of care, provide caregiver respite, and perform light housekeeping.

Assists families of individuals with disabilities to plan for the future by creating an individualized life plan and recording important personal (unique, vital and irreplaceable) information. Provides support services such as direct care, guidance and advocacy.

Provides supports and services for people with developmental disabilities under contract with the NYS Office for People with Developmental Disabilities (OPWDD), including Service Access, Brokerage, Self-Directed Services, Community Habilitation, and Family Support Services. Family Support Services include monthly youth groups, social skills classes, and summer day camps.

Case management for seniors with behavioral health needs. A team including an RN, case counselor and peer will provide confidential, comprehensive in-home assessment, develop a care plan and provide intensive short-term case management for seniors 55+ struggling with behavioral health, substance use or cognitive impairments that interfere with accessing community services and resources. Barriers to accessing services will be addressed.

This program is available to anyone in need of long term care information. Also can assist with PRI Screening (Patient Review Instrument).

InSight Shop - full service non-optical store, offering adaptive devices, visual aids, talking watches & clocks, cooking aids, large print items, frames, magnifiers, lighting, and adapted toys for sale. Vision Rehabilitation/adaptive skills training for activities of daily living. Orientation and mobility travel training. Social work/case management services. Support groups available - 3rd TU of month 10:00AM-12:00PM. Call for accessibility needs. * Offers vocational/employment services that include: assessment, placement, job supports and training, follow-up. * Offers Adaptive Technology services - to support vocational/educational goals - adaptations for computers, specialized software and hardware by referral of a New York State Commission for the Blind vocational rehabilitation counselor.* Provides children's services that include: Monthly recreation programs for children who are legally blind and their families. Parent support/education group/Family Resource Center.* Provides community education and outreach services.
