Must be a Medicaid recipient, must meet one of the following conditions: a significant mental illness two or more chronic health conditions such as asthma, diabetes, heart disease, mental health condition, or substance use disorder, must have risk factors that impact daily life functioning.
M-F 9:00AM-5:00PM. 24 hour on-call available for enrolled clients.
