Provides free Care Consultations that are confidential and personalized to the needs of individuals and families. Each of our Care Consultants has expertise and experience in the field of dementia care. Consultations can be provided by phone or video conference and can include family members in separate locations. Connect with a Care Consultant to; develop an understanding of a dementia diagnosis, identify available resources, develop strategies for navigating decisions and challenges, and to make a plan for the future .

Works to enhance the fullness of life for older adults living independently in the community. Provides services to seniors, which would help them remain in their homes, and continue to be a part of their community. Services including case management, information and referral, counseling, support groups, training and respite have been developed under Title III-E of the Older Americans Act. This program helps sustain family caregivers in their efforts to care for an older relative who has a chronic illness or disability. The Office for the Aging coordinates a variety of services. These services are designed to complement, not replace, family care. Priority is given to caregivers 60 years and older with the greatest social and economic need. A caregiver navigation program providing supportive crisis counseling, care coordination, comprehensive needs assessment, linkage to community services, education, advocacy with family or service providers, development of a caregiving plan, and follow-up for caregivers. A caregiver is an unpaid individual (whose role can be that of a spouse, partner, family member, friend, neighbor, or other) who is involved in assisting another adult with activities of daily living and/or medical tasks.
