The Independent Consumer Advocacy Network (ICAN) is an independent, conflict-free entity providing Individuals free assistance in accessing care, filing grievances, appealing adverse decisions, and understanding and exercising rights and responsibilities. There are 60 physical locations throughout NYS for in-person assistance, a call center (1-844-614-8800) for the entire state, and a website icannys.org. MEDICAL TRANSPORTATION | NASSAU COUNTY DEPARTMENT OF SOCIAL SERVICES

Provides information for those who qualify for Medical Transportation. Medicaid can pay for transportation to and from emergency medical care and services. Medicaid can also pay for transportation to and from Medicaid billable non-emergency medical care or services for a Medicaid eligible individual or reimburse that individual for the expense related to public transportation or private vehicle use. If your medical condition requires you to travel by taxi, ambulette, or ambulance, effective July 1, 2015 you must contact LogistiCare Solutions, the New York State Department of Health's transportation coordinator, at 1-844-678-1103. HCBS assists youth with a serious social, emotional and/or behavioral challenge to remain at home by offering an array of community based services to prevent placement in inpatient levels of care or Residential Treatment Facilities. Services provided would enable a child already in such a placement to be discharged more quickly. Medical Assistance - Dunkirk - Chautauqua County Department of Social Services

Administers Medicaid program to assist eligible individuals in our community in obtaining necessary and appropriate medical care through programs such as Medicaid, Child Health Plus, Medicaid Social Security Income (SSI), Personal Care Supports including nursing and personal care aids, managed care insurance, and long-term care supports, among others. Child/Teen Health Program is a preventive health program which provides continuing medical care to your children. Also provides for burial when a Temporary Assistance recipient or other indigent person dies leaving no funds or insurance sufficient to pay the cost and there are no relatives, friends or other persons liable or willing to take responsibility for the burial expense. Office serves as a Cooling Center during times of excessive heat. Always call ahead to make sure a Cooling Center is open before going there.

An organized marketplace developed by New York State to help people shop for and enroll in health insurance coverage. Public Assistance - Lockport - NCDSS

Family Assistance: Provides temporary financial support to families with children and pregnant women who are without sufficient income to maintain themselves. Safety Net: Provides vouchers for essential expenses to persons who have reached their lifetime limit of Family Assistance, to persons caring for children of non-relatives, and to single and childless couples. Temporary Disability Assistance (TDA): Provides assistance to persons with temporary disabilities, persons 60 years of age and older, caregivers of those with disabilities, and those with permanent disabilities awaiting SSI benefits. SNAP (Supplemental Nutrition Assistance Program): Provides assistance purchasing food for individuals and families with limited income. Mixed Household SNAP benefits are granted to households who have some members receiving temporary assistance and some who do not. Benefits are obtained electronically through the EBT system. Medical Assistance: Provides medical coverage to persons without insurance, premium support to persons paying a Medicare premium, and coverage for adults whose children receive Child Health Plus.
