Provides information for those who qualify for Medical Transportation. Medicaid can pay for transportation to and from emergency medical care and services. Medicaid can also pay for transportation to and from Medicaid billable non-emergency medical care or services for a Medicaid eligible individual or reimburse that individual for the expense related to public transportation or private vehicle use. HCBS assists youth with a serious social, emotional and/or behavioral challenge to remain at home by offering an array of community based services to prevent placement in inpatient levels of care or Residential Treatment Facilities. Services provided would enable a child already in such a placement to be discharged more quickly. Services offered: Planned Respite services for families and caregivers focused on enhancing their ability to support the child's mental health needs. Caregiver family support.

Medicaid provides medical assistance for those families and individuals whose medical expenses exceed their income. DSS processes applications for a variety of health insurance programs.

Provides a fund specifically designed for assistance to cancer patients requesting alternative health therapies such as nutritional counseling, natural medicines, acupuncture, chiropractic care, yoga and reflexology. Call for further information.

Administers Medicaid program to assist eligible individuals in our community in obtaining necessary and appropriate medical care through programs such as Medicaid, Child Health Plus, Medicaid Social Security Income (SSI), Personal Care Supports including nursing and personal care aids, managed care insurance, and long-term care supports, among others. Child/Teen Health Program is a preventive health program which provides continuing medical care to your children. Also provides for burial when a Temporary Assistance recipient or other indigent person dies leaving no funds or insurance sufficient to pay the cost and there are no relatives, friends or other persons liable or willing to take responsibility for the burial expense. Office serves as a Cooling Center during times of excessive heat. Always call ahead to make sure a Cooling Center is open before going there.

Persons with an active case and a medical need and with no means of transportation may get help with transportation to and from health care providers (doctor, clinic, hospital, etc.). All Temporary Assistance, Medicaid-only and SSI-Medicaid clients, may choose from five managed care plans to receive the complete menu of Medicaid Benefits. Helpline representatives assist callers with a range of issues regarding access to substance use disorder and mental healthcare issues, including helping ensure fair reimbursement of mental healthcare and substance use disorder services and fighting insurance denials of care.

An organized marketplace developed by New York State to help people shop for and enroll in health insurance coverage. Individuals, families and small businesses can use the Marketplace to help them compare insurance options, calculate costs and select coverage online, in person, or over the phone. The Marketplace also tells what type of financial assistance is available to applicants to help them afford health insurance purchased through the Marketplace. It also helps people to check their eligibility for health care programs like the Essential Plan, Medicaid, and Child Health Plus and sign up for these programs if they are eligible.

Family Assistance: Provides temporary financial support to families with children and pregnant women who are without sufficient income to maintain themselves. Safety Net: Provides vouchers for essential expenses to persons who have reached their lifetime limit of Family Assistance, to persons caring for children of non-relatives, and to single and childless couples. Temporary Disability Assistance (TDA): Provides assistance to persons with temporary disabilities, persons 60 years of age and older, caregivers of those with disabilities, and those with permanent disabilities awaiting SSI benefits. SNAP (Supplemental Nutrition Assistance Program): Provides assistance purchasing food for individuals and families with limited income. Offers Nassau and Suffolk County residents with intellectual and/or developmental disabilities and their families the ability to select services and providers that are most suited to their needs. Offers self-directed community habilitation, respite and SEMPT (supported employment). Assisted by a Fiscal Intermediary and Support Broker, individuals and family members learn how to implement HCBS (Home & Community Based) waiver services establish a habilitation plan, assess needs, and budget allocated Medicaid funds.

Provides case management services such as: assessment and referral for personal care, housekeeping and Meals-on-Wheels; screening for all entitlements and benefits such as Food Stamps, Medicaid and Senior Citizen Rent Increase Exemption (SCRIE), application assistance, advocacy.

These therapeutic, rehabilitative peer support services are open to children and youth, ages 5 through 21, who are covered by Medicaid and have mental health and/or substance use needs. These services give children and their families the power to improve their health, wellness, and quality of life. They can be provided in the home or community. Services offered include: Licensed Practitioners for in home therapy, Community Psychiatric Supports and Treatment, Psychosocial Rehabilitation, Family Peer Support and Youth Peer Advocacy and Training.
