Strives to meet the needs of people with HIV/AIDS through services from on-staff physicians, nurse practitioners, pharmacists, nurses, case managers, nutritional consultants, drug counselors, and mental health counselors as well as through partnerships with a myriad of community agencies. Comprehensive Services - HIV Medical Care: On-staff physicians and nurse practitioners are highly trained and experienced and many are faculty at the State University of New York at Buffalo Jacobs School of Medicine and Biomedical Sciences. - Primary Care: Each medical provider is also able to address the primary care needs of patients. Referrals for specialty services can be made, and most can be provided on site. - Women's HIV Services: YOU Center provides these services onsite to meet women's gynecological needs including prenatal care, with an additional focus on preventing transmission to the baby. HIV positive women are referred for appropriate and comprehensive obstetric care upon pregnancy toward safe and healthy delivery. - Adherence Pharmacology: Through a partnership with the University at Buffalo's School of Pharmacology, highly trained pharmacists are on site to offer education to providers and patients regarding disease process, treatment options, and ongoing individualized medication management. - Case Management Services: Each patient has the opportunity to meet with a medical case manager who is able to assist them with benefit management and support around their diagnosis, promoting their ongoing access to medical care. Referrals are provided to community agencies and programs as defined by the patient's need. - Rural Care Initiative: Provides HIV-infected individuals in the rural communities of Allegany, Cattaraugus, and Chautauqua Counties with access to high quality medical treatment, case management services and support services without requiring them to travel great distances. - Research Initiatives: Research staff conduct projects that examine current trends in HIV care and focus on examining the efficacy of current medical interventions, containing costs, and looking at how a person's genetics affect their medical progress. - HIV Testing: Offers free diagnostic testing service on a walk-in basis during regular business hours. Testing is also offered throughout the facility. Additionally, staff members provide community-based testing, service needs assessment, and referral linkage in high risk areas and at many community events. - Referral Basis: Patients can be referred for chemical dependency and mental health services, including psychiatric and neuropsychiatric evaluations. - Clinical Education Initiative: Offers relevant up-to-date education and training on HIV/AIDS and other related topics to community providers throughout WNY, often available for on-site training sessions tailored to meet the needs of organizations. Mini-Residency Program offers introductory and advanced clinical training for Community and Correctional Health Care Providers. Phone consultation with an HIV specialist is available 24/7 to respond to providers' needs for general clinical consultation and for the management of post exposure prophylaxis.

Provides evaluation and treatment for active tuberculosis cases, follow-up evaluation and testing for contacts, preventive treatment when required, TB skin testing, and Quantiferon blood testing. Appointments are required.

Offers classes with an evidence based curriculum (PreventT2) proven to prevent or delay the progression of prediabetes into the onset of type 2 diabetes and other serious health problems by introducing realistic lifestyle changes and goals. Prediabetes is a serious health condition where blood sugar levels are higher than normal, but not high enough yet to be diagnosed as type 2 diabetes. Trained counselors are available 24 hours a day, 7 days a week to provide information and support, enabling clients to make informed choices concerning medical, legal, and counseling needs, while offering advocacy and information about their rights. Counselors offer short-term counseling (6-8 sessions); medical, legal, and court accompaniment; crisis intervention for sexual assault survivors and their significant others; support through medical examinations and treatment; advocacy and support during interaction with law enforcement; individual consultations with professionals; community education programs; professional training programs; and referrals for additional needed services. Serves as the point of contact for domestic violence shelter placement in Wyoming County. Provides early detection, targeted testing, and preventive treatment of Latent TB Infection (LTBI). Patients diagnosed with active TB who are treated with multiple drug therapy are monitored through the Directly Observed Therapy (DOT) program. A public health nurse visits the patient five days per week to ensure that medications are taken as prescribed. Through immediate evaluation and treatment of suspected TB cases, the Department of Health is able to decrease transmission to others and prevent mass outbreaks of the disease. Evaluation and follow-up of individuals with LTBI and testing of contacts are held one day per month in the first floor clinic area of the Trott Access Center. Program works to make the community aware of the signs and symptoms of tuberculosis by providing information through media outlets, offering educational in-services to local health care facilities, and by assisting medical providers with guidelines for TB treatment and testing.
