Provides Family Care and Individual Residential Alternatives (IRA) for developmentally disabled persons ages 18 and older who are HCBW eligible. Intermediate Care Facility is available for those who are Medicaid eligible. Transportation is available for all. Supportive apartments are also available, with limited transportation. Provides respite care allowing families of developmentally disabled persons to take a break from the daily routine of care-giving duties. Currently, two locations in a certified home can offer respite for as little as one day or for a few weeks. Reservations are encouraged; however, emergency accommodations are available. Provides At-Home Residential Habilitation service that assists individuals in improving the skills necessary to live in the community while living at home. Qualified staff work one-on-one with individuals to help them learn socialization, recreation, daily living, and mobility skills.

Mental Health Community Residence is a 12-bed residence providing 24-hour care to people who are overcoming a serious mental health diagnosis. Supportive Apartments are available for those needing minimal supervision, up to 21 hours per week. Individual Residential Alternatives (IRA) offers 24 hour supervised living in various sites throughout Orleans and Genesee County.

(telephone counseling, referral, outreach and intervention/prevention for individuals and families) - Senior Services (life care planning) - Victim Assistance Program (provides services to people of all abilities who are victims of any crime, specializing in people with disabilities who have experienced domestic violence)

Provides the following services, with some transportation provided: advocacy, benefits/entitlements, developmentally disabled parent training, family reimbursement, guardianship, home care, home modification, independent living skills training, information and referral, parenting skills for parents of developmentally disabled children, residential habilitation, in-home and out-of-home respite care (day, evening, and overnight), service coordination/case management, support groups, and day camps.

Offers day habilitation services which include enhancement of daily living activities, money management, socialization, work ethics, language and communication development, and behavioral therapy

Provides residential habilitation for individuals in their own homes. Residential habilitation may include independent living skills, meal planning, money management, personal care/hygiene, and community integration.

Provides the following Clinical Services: Social Work, Occupational Therapy, Physical Therapy, Speech Language Pathology, Psychiatry, Rehabilitation Counseling, Psychology/Evaluation/Diagnosis. Other services available may include: Adaptive Aids/Equipment, Anger Management, Depression/Manic Depression Treatment, Developmental Disability Screening, Dual Diagnosis Treatment, Feeding and Swallowing Therapy, Seating and Positioning Therapy, Vocational Assessment, Vocational Counseling, Vocational Rehabilitation

Provides hourly respite and overnight respite (out of home) in planned or emergency situations (transportation is arranged whenever possible). Agency also provides residential habilitation, assistance with transportation issues, linkage, referral and assessment, intake, and advocacy for developmentally disabled children and adults living with family.

Offers a wide range of community living opportunities for people with developmental disabilities. The choice of homes includes apartment programs designed for individuals seeking greater independence and privacy. Also available is Individual Residential Alternatives (IRA) offering 24-hour support to individuals seeking group living and personalized assistance. There is also At Home Residential Habilitation services for those who live with their families but may require or request personal assistance, training or guidance within their own home.

A special Medicaid program the allows medically fragile children to be cared for at home by providing case management including evaluating and assisting with a child's therapeutic, medical, educational, and psychosocial needs, working with medical equipment and home care personnel and assessing resources to provide appropriate care within the home setting. Home adaptation and vehicle modification are also available to improve m ovement and/or safety for the child at home and to improve access from home to the community. Provides the following for individuals with developmental disabilities: - Home and Community-based Waiver - Community Habilitation - Residential Community Habilitation - In-Home Respite - Saturday Respite - Freestanding Respite (Erie County) - Self-Directed Services Staff can provide limited transportation for certain services. Aspire of WNY CILS Division can support people with developmental disabilities living in the community with family, friends, or on their own, and can assist with the exploration, planning, and implementation of community based supports. Offers a variety of supports designed to best meet an individual's needs and desires to promote, support, and maintain their active engagement in the home, work, and community.

Offers a comprehensive day habilitation program for individuals with autism and other developmental disabilities to engage in recreational and volunteer activities which offers exercise, recreation, library time, volunteer work, developing and enhancing socialization, safety skills, nutrition awareness and community integration. Also provides community habilitation where trained staff go to an individuals home to work on daily activity and other necessary skills to build as much independence as possible.

Community Habilitation services to support the child’s acquisition, maintenance, and enhancement of skills necessary to independently perform Activities of Daily Living (ADLs) and/or Health-Related Tasks.

Offers support to people with a disability in their home to develop the skills for greater independance.

RAYIM serves the developmentally disabled with in home respite for guardians of children, in home teaching of basic living skills, day trips within the community for adults, supported group home for single adults.

Home and community based independent living skills training for people with developmental disabilities through the Home and Community Based Services Waiver. Mentors work one-on-one with children and adults with developmental disabilities, using strategies and activities designed to promote skill building in the areas of communication, socialization, behavior, independent living, and movement.

Offers at home support to children and adults with developmental disabilities and autism. Provides one-on-one trained staff to assist individuals with skill development in activities of daily living, community integration and travel training.

Provides habilitation services within a person's private home or apartment. Hours provided by staff vary based on a person's needs. The main goal of this program is to increase independence while providing support to the person and the family. People are encouraged to strive to live independently in the community with minimal staff support.

Offers support to people with a disability in their home to develop the skills for greater independance.

Iindividual support and assistance for people living with family and receiving Office of People with Developmental Disabilities (OPWDD) services. Provides individualized one-on-one habilitation services in the home as well as the community. Assists the individual with increasing, acquiring, and maintaining functional skills such as independence, community integration, and self-worth.

Provides in-home training to participants that is individualized to meet their specific needs.

Provides in-home services and supports for individuals with developmental disabilities including training or assistance in areas such as personal grooming, household chores, laundry, cooking, money management, or shopping.
