Provides Family Care and Individual Residential Alternatives (IRA) for developmentally disabled persons ages 18 and older who are HCBW eligible. Intermediate Care Facility is available for those who are Medicaid eligible. Transportation is available for all. Supportive apartments are also available, with limited transportation. Provides respite care allowing families of developmentally disabled persons to take a break from the daily routine of care-giving duties. Currently, two locations in a certified home can offer respite for as little as one day or for a few weeks. Reservations are encouraged; however, emergency accommodations are available. Provides At-Home Residential Habilitation service that assists individuals in improving the skills necessary to live in the community while living at home. Qualified staff work one-on-one with individuals to help them learn socialization, recreation, daily living, and mobility skills.

Mental Health Community Residence is a 12-bed residence providing 24-hour care to people who are overcoming a serious mental health diagnosis. Supportive Apartments are available for those needing minimal supervision, up to 21 hours per week. Individual Residential Alternatives (IRA) offers 24 hour supervised living in various sites throughout Orleans and Genesee County.

A special Medicaid program the allows medically fragile children to be cared for at home by providing case management including evaluating and assisting with a child's therapeutic, medical, educational, and psychosocial needs, working with medical equipment and home care personnel and assessing resources to provide appropriate care within the home setting. Home adaptation and vehicle modification are also available to improve m ovement and/or safety for the child at home and to improve access from home to the community.
