Provides day treatment, day training, day habilitation, and employment preparation services for individuals with developmental disabilities.

Offers a variety of day services and activities for individuals with developmental disabilities including out of home respite for children to provide the caregiver a short break, social and recreational programs and in home habilitation.

In-home support services to developmentally disabled individuals who live at home with their families.

* Provides self-directed services including fiscal intermediary and broker.* Oversees residential houses providing support to persons for medical appointments, meal prep, socialization, budgeting and daily living skills.* Provides Individual Support Services (ISS) housing/rent subsidiaries for persons with I/DD.

* Provides habilitation training to individuals in their home or within their community to maintain or advance levels of independence, i.e., hygiene, household chores, mobility, personal health care, financial management, and social skills. Individuals can live independently or with family in the community.

Provides home-based services to families who have a child from birth to 5 years of age who have been identified as having a developmental delay or who is considered "at risk" for developmental delays.

Aims to provide individualized and unique counseling services to youth who are diagnosed with a developmental disability and are also experiencing mental health issues. In addition, the program offers counseling and support to family members. Can also consult with schools and other professionals who are involved in the care of the youth to ensure that treatment remains consistent across all settings.

Provides in-home services and supports for individuals with developmental disabilities including training or assistance in areas such as personal grooming, household chores, laundry, cooking, money management, or shopping.

Offers support to people with a disability in their home to develop the skills for greater independance.

Offers a comprehensive day habilitation program for individuals with autism and other developmental disabilities to engage in recreational and volunteer activities which offers exercise, recreation, library time, volunteer work, developing and enhancing socialization, safety skills, nutrition awareness and community integration. Also provides community habilitation where trained staff go to an individuals home to work on daily activity and other necessary skills to build as much independence as possible.

Offers at home support to children and adults with developmental disabilities and autism. Provides one-on-one trained staff to assist individuals with skill development in activities of daily living, community integration and travel training.

Provides residential services e.g. community residences, intermediate care facilities, individualized residential alternatives and in-home services, day habilitation, case management, supported apartments for adults and children with developmental disabilities.

Mental Health Community Residence is a 12-bed residence providing 24-hour care to people who are overcoming a serious mental health diagnosis. Supportive Apartments are available for those needing minimal supervision, up to 21 hours per week. Individual Residential Alternatives (IRA) offers 24 hour supervised living in various sites throughout Orleans and Genesee County.

Provides the following for individuals with developmental disabilities: - Home and Community-based Waiver - Community Habilitation - Residential Community Habilitation - In-Home Respite - Saturday Respite - Freestanding Respite (Erie County) - Self-Directed Services Staff can provide limited transportation for certain services. Aspire of WNY CILS Division can support people with developmental disabilities living in the community with family, friends, or on their own, and can assist with the exploration, planning, and implementation of community based supports. Offers a variety of supports designed to best meet an individual's needs and desires to promote, support, and maintain their active engagement in the home, work, and community.

Provides the following services, with some transportation provided: advocacy, benefits/entitlements, developmentally disabled parent training, family reimbursement, guardianship, home care, home modification, independent living skills training, information and referral, parenting skills for parents of developmentally disabled children, residential habilitation, in-home and out-of-home respite care (day, evening, and overnight), service coordination/case management, support groups, and day camps.

A special Medicaid program the allows medically fragile children to be cared for at home by providing case management including evaluating and assisting with a child's therapeutic, medical, educational, and psychosocial needs, working with medical equipment and home care personnel and assessing resources to provide appropriate care within the home setting. Home adaptation and vehicle modification are also available to improve m ovement and/or safety for the child at home and to improve access from home to the community. *Offers long term support and education to parents with developmental disabilities.

• Families may be headed by one or two parents but must have custody of the child.

*Designed for four families currently, two of them rent safe, affordable Valley Commons homes that serve as learning environments for the parents.

*Offers parenting and other classes for parents that are not enrolled in the program.

Provides support to families to help relieve them of their caregiving duties.

Provides individualized one-on-one habilitation services in the home as well as the community. Assists the individual with increasing, acquiring, and maintaining functional skills such as independence, community integration, and self-worth.

Provides day habilitation and community habilitation services to individuals with developmental disabilities. Services provided in a group or 1:1 setting.

JM Murray's Recreation Respite Program offers recreational activities to individuals with developmental disabilities while providing respite for families. Organize, coordinate, and accompany people on day trips and overnight trips. This a non-waiver service through the Office of People with Developmental Disabilities (OPWDD). Individuals must live at home with a caregiver and also receive OPWDD services.
