This waiver program assesses, plans, and supports the transition of nursing home and long term care facility residents, who are eager to and capable of returning to community living, back to integrated community living settings of their own choosing. It also assists persons at risk of being placed in a skilled nursing facility. WILC is the Hudson Valley Region Center providing services for the residents

Skilled nursing and rehabilitation facility of 182 beds providing long term care, hospice care, occupational, physical and speech therapy, pain management, respiratory therapy, IV therapy, neurobehavioral unit, tracheostomy care, traumatic brain injury care, wound care and short-term rehabilitation.

Offers professional/skilled nursing care and outpatient therapy. Provides physical therapy, occupational therapy, speech pathology, aquatic therapy, and social services.

Wesley Health Care Center has committed one of our units to residents in the two earlier stages of chronic dementia. By providing a therapeutic environment to sustain a resident with a dementia diagnosis, these folks benefit from proactive programming designed to keep them stimulated and alert through all aspects of life as their ability allows.

On-site, residential rehabilitation services from licensed and certified physical, occupational and speech therapists, plus therapy assistants for individuals with all levels of physical limitations.Enables residents to reach the highest level of functioning possible, so they may be able to return home or live at a lower level of assisted care.

Promotes and provides access to appropriate and cost effective long term care services for all residents of Saint Lawrence County. Provides information and referral services to consumers and providers regarding long term care and supports.
