A home visiting program for Chautauqua County mothers and their children from pregnancy to age 2. Helps families to achieve healthy outcomes through an evidence-based home visiting curriculum, developmental screenings for children, screenings for mothers, and resources and referrals to address social determinants of health. Provides the following services: - Education on health topics and private supportive visits in the comfort of the family's home - Linkage with nutrition programs and breastfeeding supports - Assistance finding a primary care doctor, dentist, physicians who can address any current health concerns, and medical/health insurance - Assistance applying for WIC - Assistance obtaining family planning and birth control - Linkage to other services in the community - Assistance scheduling appointments for mothers and children - Assistance with transportation needs and translation - Advocacy

Provides case management and prenatal/postnatal home visiting services. Utilizes trained Community Health Workers (CHW) to identify, recruit, and assess the needs of targeted women before, during, and after their pregnancy. CHWs provide women with referrals for services needed, education regarding the benefits of family planning and early prenatal care, assistance with obtaining health insurance and medical care, and follow-up to assure that services were secured. Additional services include breastfeeding support and a smoking cessation program.

Comprehensive program that deals with families at risk of child abuse and/or child neglect. Provides weekly home visits, child development services, and positive parenting training, along with case management services when needed.

An evidence-based community health program that serves low-income women pregnant with their first child. Each vulnerable new mother is partnered with a registered nurse early in her pregnancy and receives a home visit from them. NFP nurses work with mothers to help them: - Have a healthy pregnancy and a healthy baby - Become a better parent - Build a strong network of support - Make their home a safe place for the baby to live and play - Find healthcare, childcare, job training, and other support services available in the community NFP nurses also work to help mothers continue their education, develop job skills, set goals for their family's future and find ways to reach them.

Home visiting nursing program that provides prenatal and postnatal supportive services. MOMS is designed to improve equitable access to quality maternal child health care, increase lactation support for parents choosing to breast feed, and enhance the coordination of care with other community resources. Program services include: Health Screening & Monitoring, Pregnancy Education, Nutrition Screening & Monitoring, Infant Care Education, Lactation Counseling, Postpartum Mental Health Education, Childbirth and Family Planning, Early Childhood Development, Community Resource Referrals, and help with WIC program enrollment (if appropriate) and making an appointment with a participating OBGYN. Staff RN is also a Certified Lactation Consultant (CLC) who can answer questions about yourself and your newborn in regard to breast/chest feeding.
