Priscilla Project - Jericho Road Community Health Center

Serves low-income pregnant women, including those who are refugees. Program matches particularly high-risk participants with one-on-one mentors as early in their pregnancy as possible. Mentors are volunteer women recruited from the local community who are expected to visit their mentees at least one hour weekly and to provide mentorship until three months post-delivery. Mentors provide education, friendship, and transportation to prenatal appointments, and assist mentees in getting baby supplies. Mentors also provide critical social support to culturally isolated women. In addition, participants are provided with doula services, educational classes, and referrals to other support agencies. Cultural- and language-matched doulas provide physical, emotional, and informational support to mothers, assist in creating an individual birth plan, and provide interpretation services for refugees and staff during labor and delivery. Clients are screened for WIC and SNAP assistance. Clients are also eligible to attend educational classes on subjects including breastfeeding, nutrition, and the birth experience.

Healthy Families Allegany/Cattaraugus (HFAC) - Parent Education Program

Program offers home-based services to expectant families and new parents, beginning during the prenatal period or shortly after the birth of the child. Participating families are screened to identify risk factors and stresses that they may face, and are offered long-term in-home services until the child enters school or Head Start.

MOMS Program - Cattaraugus County Health Department

Home visiting nursing program that provides prenatal and postnatal supportive services. MOMS is designed to improve equitable access to quality maternal child health care, increase lactation support for parents choosing to breast feed, and enhance the coordination of care with other community resources. Program services include: Health Screening & Monitoring, Pregnancy Education, Nutrition Screening & Monitoring, Infant Care Education, Lactation Counseling, Postpartum Mental Health Education, Childbirth and Family Planning, Early Childhood Development, Community Resource Referrals, and help with WIC program enrollment (if appropriate) and making an appointment with a participating OBGYN. Staff RN is also a Certified Lactation Consultant (CLC) who can answer questions about yourself and your newborn in regard to breast/chest feeding. Visiting Nursing Association of WNY - Jamestown Office

Services include nursing, social services, home health aides, social workers, infusion therapy, home health evaluations and assessments, nutritional counseling, arranging of medical equipment and supplies, and physical, occupational, and speech therapy. Specialized programs in pediatrics, behavioral health, and wound care available. In-facility assessments for discharge planning at hospitals and skilled nursing facilities available upon request. Serves as a state provider in the MOMS (Medicaid Obstetrical & Maternal Services) Program, providing comprehensive support nursing services for prenatal and postpartum women.

Welcome Home Baby Program - Chautauqua County Health Department

Promotes healthy babies and families by providing every new parent in the county a one-time visit with one of our public health nurses. During the visit, parents receive one-on-one education related to infant safety, feedings, development, immunizations, post-partum depression, and more.

Healthy Families Niagara - Pinnacle Community Services

A program designed to help parents meet the challenges of parenting young children and to ensure their children's healthy development. Program provides comprehensive, home-based services to families during pregnancy or with newborns up to three months of age. The family can continue in the program until the child is five years old or enters Head Start. Services of a Certified Lactation Consultant, Doula, and/or Father Advocate can be included as a complimentary add-on for families. A support group for fathers is also offered.

Tuscarora Health Center - NFMMC

A comprehensive health center offering primary care (assessment, treatment, and referral), dental clinic, health education, and diabetes education. Tuscarora Community Health Worker Program provides home-based case management services to participating women and their children and helps them overcome barriers to care. Workers help identify complicating factors that make it difficult for clients to access the care they need; assist in enrolling pregnant women into prenatal care and children into primary care; assist in enrolling for services such as WIC, Medicaid/PCAP and Child Health Plus; provide education on topics such as breastfeeding, baby care, lead poisoning, immunization, nutrition, HIV/AIDS, domestic violence, and substance abuse; provide referrals to food pantries, housing assistance, educational programs, childcare, and other services; and provide support and help to overcome the barriers that prevent families from receiving a healthy start in life. HOURS: Monday, 9am-6pm; Tuesday-Friday, 9am-5pm. Tuscarora Community Health Workers Program: Monday-Friday, 8:30am-4pm.

Maternal & Infant Health Collaborative- Chautauqua County Health Department

Provides case management and prenatal/postnatal home visiting services. Utilizes trained Community Health Workers (CHW) to identify, recruit, and assess the needs of targeted women before, during, and after their pregnancy. CHWs provide women with referrals for services needed, education regarding the benefits of family planning and early prenatal care, assistance with obtaining health insurance and medical care, and follow-up to assure that services were secured. Additional services include breastfeeding support and a smoking cessation program.
