A comprehensive health center offering primary care (assessment, treatment, and referral), dental clinic, health education, and diabetes education. Tuscarora Community Health Worker Program provides home-based case management services to participating women and their children and helps them overcome barriers to care. Workers help identify complicating factors that make it difficult for clients to access the care they need; assist in enrolling pregnant women into prenatal care and children into primary care; assist in enrolling for services such as WIC, Medicaid/PCAP and Child Health Plus; provide education on topics such as breastfeeding, baby care, lead poisoning, immunization, nutrition, HIV/AIDS, domestic violence, and substance abuse; provide referrals to food pantries, housing assistance, educational programs, childcare, and other services; and provide support and help to overcome the barriers that prevent families from receiving a healthy start in life. Works to provide the education, support, and care needed by families (especially those in Black and Brown communities) to have a healthy start for their babies. Once enrolled, clients have access to care through home visits from community health workers (CHWs) and registered nurses, who will work with a client's chosen obstetrics provider or help connect them with one. Nurses visit clients throughout their pregnancy to deliver care and answer questions whenever needed. Community health workers support areas of life that affect clients' health, like housing, food, transportation, baby supplies, and preparation for the baby's arrival. Provides both one-on-one and group educational support so that clients know what to expect at every stage of their parenthood journey. Partners with healthcare providers, doulas, and community organizations to improve outcomes for pregnant persons and their families.

An evidence-based community health program that serves low-income women pregnant with their first child. Each vulnerable new mother is partnered with a registered nurse early in her pregnancy and receives ongoing home visits from the nurse. NFP nurses work with mothers to help them: - Have a healthy pregnancy and a healthy baby - Become a better parent - Build a strong network of support - Make their home a safe place for the baby to live and play - Find healthcare, childcare, job training, and other support services available in the community NFP nurses also work to help mothers continue their education, develop job skills, set goals for their family's future and find ways to reach them.

Provides parenting workshops, toddler story hours and playgroups, parent support groups, book clubs, home visiting, and referrals to community resources.

Provides outreach and home visitation to high-risk women, men, and their families. Offers assistance and advocacy for clients to overcome barriers to accessing needed services including help finding a doctor or other medical provider, help with enrollment in a health insurance plan, information on reproductive health, and referrals for help with issues such as relationship violence, substance abuse, and mental health.

Provides preschool children with early learning experiences that are aimed at meeting their developmental, social, and physical needs prior to entering elementary school.

Provides a range of in-home services including elderly care, HIV/AIDS management, home health aides, home infusion therapy, in-home nursing, maternal child health, medication dispensing services, occupational therapy, palliative care, pediatrics, physical therapy, post-surgical care, social work, speech-language pathology, and wound care.

Provides evidence-based, accredited parent support and early intervention program that promotes health and development of expectant parents and parents of babies under three months of age. Offers long-term, home-based partnerships between the Family Specialist and parents from pregnancy until the child reaches school age.

Offers home visits to pregnant women for prenatal health education and guidance. Facilitates connections to additional programs and services that may help the individual through the pregnancy including referring agencies that can help in enrolling into Medicaid for uninsured pregnant individuals. Offers home visits by Certified Lactation Counselors following delivery to assist the breastfeeding mother with issues or concerns.
