Offers assistance and advocacy for clients to overcome barriers to accessing needed services including help finding a doctor or other medical provider, help with enrollment in a health insurance plan, information on reproductive health, and referrals for help with issues such as relationship violence, substance abuse, and mental health.

Services are offered from pregnancy until their child reaches age five. Home visitors bring toys, books, activities, and information to stimulate your child's development. They also complete developmental screenings, monitoring your child's milestones, and offer referrals to community resources. Home visitors are certified by the Parents As Teachers National Center as Parent Educators and use the Parents as Teachers curriculum.

PACT provides parent education program focusing on parent as primary teacher of young child.

Provides evidence-based, accredited parent support and early intervention program that promotes health and development of expectant parents and parents of babies under three months of age. Offers long-term, home-based partnerships between the Family Specialist and parents from pregnancy until the child reaches school age.

Promotes healthy babies and families by providing every new parent in the county a one-time visit with one of our public health nurses. During the visit, parents receive one-on-one education related to infant safety, feedings, development, immunizations, post-partum depression, and more.

A home visiting program for Chautauqua County mothers and their children from pregnancy to age 2. Helps families to achieve healthy outcomes through an evidence-based home visiting curriculum, developmental screenings for children, screenings for mothers, and resources and referrals to address social determinants of health. Provides the following services: - Education on health topics and private supportive visits in the comfort of the family's home - Linkage with nutrition programs and breastfeeding supports - Assistance finding a primary care doctor, dentist, physicians who can address any current health concerns, and medical/health insurance - Assistance applying for WIC - Assistance obtaining family planning and birth control - Linkage to other services in the community - Assistance scheduling appointments for mothers and children - Assistance with transportation needs and translation - Advocacy

A comprehensive health center offering primary care (assessment, treatment, and referral), dental clinic, health education, and diabetes education. Tuscarora Community Health Worker Program provides home-based case management services to participating women and their children and helps them overcome barriers to care. Workers help identify complicating factors that make it difficult for clients to access the care they need; assist in enrolling pregnant women into prenatal care and children into primary care; assist in enrolling for services such as WIC, Medicaid/PCAP and Child Health Plus; provide education on topics such as breastfeeding, baby care, lead poisoning, immunization, nutrition, HIV/AIDS, domestic violence, and substance abuse; provide referrals to food pantries, housing assistance, educational programs, childcare, and other services; and provide support and help to overcome the barriers that prevent families from receiving a healthy start in life.
