From resolving medical debts to proving eligibility for public health insurance, obtaining services that should be covered by Medicaid, and helping with language or disability accommodations, helps to eliminate barriers to healthcare for people and families with low-income.

Offers parent advocacy, educational advocacy, and tutoring to help children and young adults with developmental disabilities and their families to navigate medical and educational concerns.

Care coordinators assist clients with: - Finding a doctor or specialist - Making sure that doctors are talking to each other and the client about their care - Advocating for the client - Helping the client understand and remember confusing information - Scheduling appointments, tests, and more - Getting care when the client is discharged from the hospital - Getting into mental health counseling or substance use programs - Finding transportation to medical appointments

Provides assistance to children from birth to 21 years of age who have (or may have) a serious or chronic physical, developmental, behavioral, or emotional condition in locating a physician or other health care provider and may also provide assistance in obtaining insurance and/or other programs that help to cover health care costs. Supportive measures as well as medical information may be provided.

Cancer nurse navigators: guide patients and families through cancer care: screening, diagnosis, treatment and support services act as patient advocates, interpreters and educators serve as liaisons between patients, caregivers and service providers

Peer Advocates provide an outreach program for recipients of mental health services in the community and for patients receiving treatment at Niagara Falls Memorial Medical Center in the Behavioral Health Service Unit. Peer-Linkage Advocates work with patients, family members, and outside sources, providing reassurance and notifying patients of all available services. The goal of Peer Advocates is to see that each patient has access to the best and most suitable resources. Information & Resources available include: - Self-help - Social clubs - Information on legal rights for psychiatric patients - Self-help groups - Referrals to community services and programs Peer Advocates: - Work with people who have psychiatric diagnoses - Work with friends and family members to help them to understand the process in the hospital - Keep clients informed about all available services - Make sure that the client will receive the best services and care available Peer Advocates are limited to helping admitted patients at NFMMC only. The Peer Advocacy Program also caters to educational needs and has established an advocacy information center and network.

Provides recreational activities for children. Summer residential camp provided for children ages 5-12 years old.

Guides and supports participants from conditions of illness, risk and dependence to conditions of wellness, safety and independence, while addressing the unique needs of their HIV diagnosis. Supports access to affordable housing options, case management, connection to substance use treatment, mental health programs, support with health care and medical compliance, and community resources, nutrition programs, and transportation.

Breast Cancer Screening - Patient Navigation: These programs help women, regardless of insurance status, to overcome barriers to access and complete breast cancer screening at breast centers accredited through the National Accreditation Program for Breast Centers (NAPBC). New York State Breast and Prostate Cancer Peer Education: These programs offer education for women about the importance of regular breast cancer screening, and, for men, about the risks of prostate cancer. Provides a statewide breast cancer hotline offering information, referrals, and support groups for women and men with breast cancer. Services include: a toll-free Breast Cancer Hotline for New Yorkers with and/or concerned about breast cancer and is staffed by professionally trained and supervised volunteers, all of whom are breast cancer survivors, and is available daily from 9am to 9pm. We provide licensed Social Work Services (in English and Spanish) which includes counseling, crisis intervention and referrals, in person, on the phone and via zoom. The program offers Support Groups which are facilitated for individuals and include groups for newly diagnosed, women with metastatic or Stage 4 disease, women under 40, Creative Arts, recurrence of breast cancer; we also provide two Spanish-language support groups: Cafecito which is a Survivorship group and Para Nuestro Bienestar for newly diagnosed women. Sisters United in Health (SUIH) is a coalition of local organizations that provides community outreach, health education, workshops and referrals for free or low-cost mammography and other health screenings to underserved communities.
