* Provides Health Home services through a group of health care and service providers. Enrolled individuals will work with a care manager to develop an individualized care plan.* Assists individuals with: Coordinating care through communication with other providers. Advocating on their behalf to receive care. Navigating the healthcare system. Connecting to mental health and substance abuse providers. Connecting to needed medications. Linking to resources for finding affordable housing, benefits, food, clothing, childcare, transportation. Learning more about activities to improve health. Identify goals and ensure all providers involved in care understand the goals and care plan.

Provides volunteers to accompany older adults to medical appointments and procedures. They are a patient's second eyes and ears to help them correctly follow their doctor's instructions, including discharge plans if hospitalized.

Offers parent advocacy, educational advocacy, and tutoring to help children and young adults with developmental disabilities and their families to navigate medical and educational concerns.

Provides information, referral, and advocacy for children who may require health or related services of a type or amount beyond that typically required by children. Case management is also provided which can connect families to appropriate services in the community. Services include: accessing health insurance for the child; aid in determining if the child may be eligible for Medicaid under new State programs, or in some cases, SSI benefits; assistance in obtaining medical services or equipment at home; providing advocacy with other service providers to ensure quality of care for the child; developing a support network.

Provides help for individuals and families to get connected to health insurance, finding a primary care physician or other doctor, prepare for doctors visits, and coordinate transportation.

Provides medical and radiation oncology services along with supportive services for individuals with cancer.

Assists youth and adults who have CDPHP/Medicaid insurance in attaining primary preventative care.

* Works with high-need women and families to access preventive and primary health care services, with an emphasis on pregnant women who are not enrolled in health care or supportive services.

Guides and supports participants from conditions of illness, risk and dependence to conditions of wellness, safety and independence, while addressing the unique needs of their HIV diagnosis. Supports access to affordable housing options, case management, connection to substance use treatment, mental health programs, support with health care and medical compliance, and community resources, nutrition programs, and transportation.
