Provides volunteers to accompany older adults to medical appointments and procedures. They are a patient's second eyes and ears to help them correctly follow their doctor's instructions, including discharge plans if hospitalized.

Provides help for individuals and families to get connected to health insurance, finding a primary care physician or other doctor, prepare for doctors visits, and coordinate transportation.

Provides a statewide breast cancer hotline offering information, referrals, and support groups for women and men with breast cancer. Services include: a toll-free Breast Cancer Hotline for New Yorkers with and/or concerned about breast cancer and is staffed by professionally trained and supervised volunteers, all of whom are breast cancer survivors, and is available daily from 9am to 9pm. We provide licensed Social Work Services (in English and Spanish) which includes counseling, crisis intervention and referrals, in person, on the phone and via zoom. The program offers Support Groups which are facilitated for individuals and include groups for newly diagnosed, women with metastatic or Stage 4 disease, women under 40, Creative Arts, recurrence of breast cancer; we also provide two Spanish-language support groups: Cafecito which is a Survivorship group and Para Nuestro Bienestar for newly diagnosed women. Sisters United in Health (SUIH) is a coalition of local organizations that provides community outreach, health education, workshops and referrals for free or low-cost mammography and other health screenings to underserved communities. Works to enhance the fullness of life for older adults living independently in the community. Provides services to seniors, which would help them remain in their homes, and continue to be a part of their community. Services include friendly visits, companionship assistance, transportation and shopping assistance, spiritual care, health education and counseling, network to community agencies, advocacy for quality care.

Offers breast cancer survivors the opportunity to meet one-on-one with certified breast cancer navigator. Assesses individual's emotional and educational needs. Provides information on breast health, treatment options and assists in making referrals to appropriate medical and community resources. Meets the first Wednesday of each month. Helps families improve the health and mental health care services they receive for their children by providing case management services that address barriers parents may encounter and provide support navigating the maze of systems.

Information and referral program for children 0-21 years of age who have chronic conditions or are developmentally delayed, and their families. Staff can assist with various activities including assistance with application processes and appointments.

Care coordinators assist clients with: - Finding a doctor or specialist - Making sure that doctors are talking to each other and the client about their care - Advocating for the client - Helping the client understand and remember confusing information - Scheduling appointments, tests, and more - Getting care when the client is discharged from the hospital - Getting into mental health counseling or substance use programs - Finding transportation to medical appointments

Provides information, referral, and advocacy for children who may require health or related services of a type or amount beyond that typically required by children. Case management is also provided which can connect families to appropriate services in the community. Services include: accessing health insurance for the child; aid in determining if the child may be eligible for Medicaid under new State programs, or in some cases, SSI benefits; assistance in obtaining medical services or equipment at home; providing advocacy with other service providers to ensure quality of care for the child; developing a support network.

Offers parent advocacy, educational advocacy, and tutoring to help children and young adults with developmental disabilities and their families to navigate medical and educational concerns.

Peer Advocates provide an outreach program for recipients of mental health services in the community and for patients receiving treatment at Niagara Falls Memorial Medical Center in the Behavioral Health Service Unit. Peer-Linkage Advocates work with patients, family members, and outside sources, providing reassurance and notifying patients of all available services. The goal of Peer Advocates is to see that each patient has access to the best and most suitable resources. Information & Resources available include: - Self-help - Social clubs - Information on legal rights for psychiatric patients - Self-help groups - Referrals to community services and programs Peer Advocates: - Work with people who have psychiatric diagnoses - Work with friends and family members to help them to understand the process in the hospital - Keep clients informed about all available services - Make sure that the client will receive the best services and care available Peer Advocates are limited to helping admitted patients at NFMMC only. The Peer Advocacy Program also caters to educational needs and has established an advocacy information center and network.

A one-stop center for health and social services with the goal of improving quality of life for residents of Niagara Falls living under the poverty line by increasing access to support resources and bringing human service providers together in a central location. Services available at Daybreak include shower and laundry facilities, homeless outreach/case management, housing related coordinated entry, forms/application assistance, rent payment assistance (currently unavailable), security deposit payment assistance (currently unavailable), and emergency housing vouchers for those who are selected via lottery from NYS Homes and Community Renewal. Provides access to a computer room, printer, office space, and technology for tele-health and other virtual services. Free haircuts available for those in need every 2nd, 3rd, and 4th Monday of each month, 10am-12 noon; appointments strongly encouraged. Also provides access to housing resources within the WNY Continuum of Care: a collaborative program that provides a full range of emergency, transitional, and permanent housing for homeless persons. The team at Daybreak helps households at risk of homelessness or housing instability by providing services such as housing referrals, rental arrears, utility arrears, and affordable housing recommendations. Additionally, a Community Health Nurse from Mount St. Mary’s Hospital is present at Daybreak to provide free health and wellness guidance to at-risk individuals. The Nurse acts as a personal health care coordinator for guests by helping to monitor chronic health problems, offering personalized guidance, giving referrals to appropriate health care providers and resources, and coordinating vaccines, flu shots, and health screenings through Mount St. Mary’s and the Niagara County Health Department.

Seeks to improve the system of care for children and youth with special health care needs and their families. The program helps shape public policy so families can get the best health care for their children. A diagnosis is NOT required. Services include: - Matching families with community resources and referring to community programs - Advocacy with health care providers and school districts - Linkage to medical providers and specialists - Support and play groups - Transportation services - Translation and interpretation services - Transition to adult services and more

Linking to resources for finding affordable housing, benefits, food, clothing, childcare, transportation. Learning more about activities to improve health.
