This program consists of a combination of programs and services designed to improve education and access for community members working toward optimal health and well-being. In partnership with health care providers, Community Health Workers provide intake, referral, navigation and case management services to patients at Primary Care Practices.

Certified counselors provide free, accurate, and objective information, counseling, assistance and advocacy on Medicare, private health insurance, Medicaid and related health coverage plans including Part D prescription drug plans.

The Health Insurance Information Counseling and Assistance Program (HIICAP) supplies people over 60 with counseling regarding Medicare A, B and D, QMB, SLMB, retiree health plans, other health insurance, medigap policies, long term care insurance, claim processing, EPIC, etc. THe Office for the Aging needs volunteers for its health insurance counseling helpline. To become a volunteer, one must go through HIICAP training

We provide Medicaid and Medicare (A,B,C,D) benefits counseling, EPIC, Lifeline telephone discount, low income subsidy, part D prescription benefits, Medicare Supplemental insurance, Employer insurance benefits, Veterans benefits, Medicare Improvements for Patients and Providers Act (MIPPA), Medicare Savings Program, and more.

Educates seniors about Medicare plans and Buy-In programs, prescription plans, Medigap plans, New York State Elderly Prescription Drug Assistance Program (EPIC), and Medicaid related plans. Offers seniors the ability to call regarding any health care issues that they may encounter with their current situation.

Offers assistance to enroll in government-sponsored health insurance programs, such as Medicaid, Child Health Plus and Family Health Plus. Call to find out if you may be eligible.

Offers assistance for anyone who needs assistance regarding health insurance issues, along with social workers who can coordinate and support individuals with their medical care needs.

Senior Benefits Information Centers (SBICs) help adults 60 and older, and those caring for the elderly, achieve a better understanding of Medicare health plans and prescription drug coverage (Part D prescription drug coverage, Medicare Advantage, Medic-Gap). Information and counseling is provided about different types of government programs, including federal and state financial assistance programs, and county-wide benefits. You can meet one-on-one with a trained counselor at one of our library-based locations. Spanish interpreter can be made available by appointment.

Open Door provides Outreach and Enrollment staff that are committed to enrolling people in insurance programs that fit their budgets. Coverage is often available regardless of citizenship or immigration status.

Assists with completing ADAP (AIDS Drug Assistance Program), ADAP Plus, Home Care, APIC and other health insurance applications; and evaluates current health care plans to ensure HIV+ people and the general community are in the plan that best fits their needs. Outreach Specialists will assist in finding comprehensive health insurance which includes medical and prescription coverage.

Certified Application Counselors are available to assist patients access medical services by providing health insurance application and enrollment assistance via the New York State of Health Marketplace.

NY State of Health is an organized marketplace developed by New York State to help people shop for and enroll in health insurance coverage. Enrollment for 2024 is open, enroll or renew by December 15 for coverage that starts on January 1.

Provides help and support in accessing, medical and dental care, specialty care, housing, social security disability income, supplemental social security income, Medicaid, AIDS drug assistance programs, emergency financial assistance, mental health treatment, legal assistance including health care proxy, living wills, and permanency planning.
