FAMILY HEALTH PLUS | NEW YORK STATE DEPARTMENT OF HEALTH

Provides free, comprehensive health coverage to adults who are ineligible for Medicaid coverage because their incomes and/or available resources exceed the Medicaid limits. Coverage is provided by managed care plans and includes physician services, inpatient and out-patient hospital care, prescription drugs, and smoking cessation products, lab tests and x-rays, vision, speech and hearing services, rehabilitation services, limited durable medical equipment, emergency room and emergency ambulance services, drug, alcohol, and mental health treatment (limited), diabetic supplies and equipment, radiation therapy, chemotherapy, hemodialysis, and dental services (if offered through plan). Annual recertification by mail or phone.

All services are free to participants, who must be New York State residents, 19 through 65 years old, not eligible for Medicaid, meet income criteria, a citizen or qualified immigrant without insurance coverage. Help provided in choosing a health plan provider.
