Provides a caring community that understands the challenges of living with diabetes. Helps individuals to obtain knowledge, skills, and support to better manage prediabetes and diabetes. Programs include: - Diabetes Prevention Program: For those with prediabetes or at high risk for type 2 diabetes. A year long program focused on implementing lifestyle changes to reduce the risk of developing type 2 diabetes. - Diabetes Counseling: For individuals with prediabetes or diabetes. Provides one-on-one visits with a diabetes care and education specialist to discuss individual needs and support. - Diabetes Self-Management Classes: For any individual with diabetes. Classes focus on the fundamentals of diabetes self-management, covering topics such as understanding diabetes, blood sugar monitoring, nutrition and exercise, acute and chronic complications, and diabetes medications. - Diabetes Support Group: For any individual with diabetes and those supporting them. Meets once every other month to share struggles, celebrate successes, and discuss topics related to diabetes management.

Offers outpatient programs and services that provide education, resources, guidance, and encouragement to help patients regain their health and successfully manage their conditions. Eddy SeniorCare is a Program of All-Inclusive Care for the Elderly (PACE) that integrates medical and long-term healthcare management to help chronically ill adults live independently and avoid unnecessary nursing home placement. Eddy SeniorCare offers an interdisciplinary care team working together to manage each senior's medical conditions and provides day services, nursing care, rehabilitation, social work, transportation, recreation, meals, personal care and at-home services.
