Individual, group, and Telehealth diabetes self-management education support services provided to meet the personal needs and goals of individuals with diabetes, whether newly diagnosed or having long-term diabetes. Program is "ADA Recognized," meeting very strict ADA National Diabetes Self-Management Education and Support standards. Faculty members are Certified Diabetes Educators.

Offers outpatient programs and services that provide education, resources, guidance, and encouragement to help patients regain their health and successfully manage their conditions. Plans and implements wellness health screens and educational programs for health professionals and the community. These programs and services are offered in response to community need. All health screen participants receive follow-up and assistance with referral, if needed. Screenings and programs include a tobacco-free program, blood pressure screenings, free diabetes screenings, and disease management programs.

Offers chronic disease and pain self-management workshops to assist individuals living with ongoing or limiting health conditions or individuals who care for them. Participants learn self-management techniques and skills needed in day-to-day management of any type of ongoing health condition.

Eddy SeniorCare is a Program of All-Inclusive Care for the Elderly (PACE) that integrates medical and long-term healthcare management to help chronically ill adults live independently and avoid unnecessary nursing home placement. Eddy SeniorCare offers an interdisciplinary care team working together to manage each senior's medical conditions and provides day services, nursing care, rehabilitation, social work, transportation, recreation, meals, personal care and at-home services.

Sessions meet for 2.5 hours in local community settings across the region. Participants will:

• Gain support from peers that have similar conditions
• Identify stressors and learn new coping skills
• Learn to talk to family, friends, and health care professionals about their chronic conditions
• Find practical ways to deal with pain and fatigue
• Discuss ways to maintain and improve strength, flexibility, and endurance
• Set goals and make a step-by-step plan to improve health and life

* Provides a Diabetes Self-Management (DSM) program "Living Well with Diabetes," a free 2.5-hour workshop once a week for 6 weeks. Participants will gain support to:

• Manage symptoms
• Communicate effectively with doctors and other healthcare professionals
• Ease frustration

• Eat healthier and read nutrition labels
• Keep blood sugar in check
• Make daily tasks easier

Provides medical services to treat acute and chronic illnesses for men, women, and children. Offers behavioral health services to primary care patients, including counseling, psychiatric assessments, and medication, as well as medication-assisted treatment for substance use disorders, including opioid treatment programs. Provides insurance enrollment assistance.

Provides various health services including disease self-management programs and rehabilitation for individuals recovering from an injury or surgery. Also provides a social worker to help individuals having difficulties coordinating services or accessing care.

Provides a six (6) week, evidence-based workshop for people living with ongoing or limiting health conditions or people who care for them. Participants learn self-management techniques and skills needed in day-to-day management of any type of ongoing health condition. Workshop topics include: • How to deal with frustration, fatigue, isolation, and pain • Maintaining a mind-body connection • Ways to maintain and improve strength, flexibility, relaxation, and endurance • Managing & maintaining healthy eating habits • How to communicate more effectively with family, friends, and health professionals
