Provides easy, local access to information and assistance for consumers and their families who are exploring available options for long term care. An effort is made to provide all available care options so as to enable consumers to remain at home. Also conducts screenings to determine eligibility for different services. Will conduct in-home assessments as necessary. Serves as the point of contact for the Senior Farmers' Market Nutrition Program, which provides low-income seniors with coupons that can be exchanged for eligible foods (fruits, vegetables, honey, and fresh-cut herbs) at farmers' markets, roadside stands, and community supported agriculture (CSA) programs.

Offers information and assistance, education, and screening for older adults ages 60 and older, adults and children with disabilities, as well as family members, caregivers, and professionals. Assists clients in making informed choices regarding their long term care needs by offering information on which providers offer needed services nearest to them and helping clients contact these providers. A statewide, locally based point of entry system that provides free, one stop access to local community services and supports for the elderly, disabled, and/or developmentally disabled, including children with special needs and their caregivers. Provides assistance with exploring long term care options and makes referrals for Meals on Wheels and Personal Emergency Response buttons. Assisting seniors 65+ securing a COVID-19 vaccination.

Provides information, assistance, and access to long term supports and services for individuals of all ages with a single phone call. Assists with applications for SNAP, HEAP, and medical insurance (Medicaid & NYSOH Market Place). Independent Living Centers and Senior Services/Office of the Aging work together to help seniors, caregivers, and individuals with disabilities access services and programs through information and referral and person-centered options counseling.

Serves older adults who are blind or visually impaired by offering the opportunity for stimulation and companionship via adapted activities. Allows seniors to leave their homes and be socially and physically active while being monitored and supervised by a professional team.

Serves older adults who are blind or visually impaired by offering a full array of customized service programs ranging for benefits assistance to health and wellness programs in a fully accessible facility. Also offers a hot dinner meal Monday-Friday.

Maintains a unique multi-media resource center filled with the latest books, DVDs and articles on dealing with Alzheimer's disease and caring for persons with Alzheimer's disease.
Resources are accessible at the Albany office and at the Warren County Office for the Aging.
