A statewide, locally based point of entry system that provides free, one stop access to local community services and supports for the elderly, disabled, and/or developmentally disabled, including children with special needs and their caregivers. Provides assistance with exploring long term care options and makes referrals for Meals on Wheels and Personal Emergency Response buttons. Assisting seniors 65+ securing a COVID-19 vaccination.

Assists with applications for SNAP, HEAP, and medical insurance (Medicaid & NYSOH Market Place). Independent Living Centers and Senior Services/Office of the Aging work together to help seniors, caregivers, and individuals with disabilities access services and programs through information and referral and person-centered options counseling.

Serves older adults who are blind or visually impaired by offering the opportunity for stimulation and companionship via adapted activities. Allows seniors to leave their homes and be socially and physically active while being monitored and supervised by a professional team.
