Description

A statewide, locally based point of entry system that provides free, one stop access to local community services and supports for the elderly, disabled, and/or developmentally disabled, including children with special needs and their caregivers. Provides assistance with exploring long term care options and makes referrals for Meals on Wheels and Personal Emergency Response buttons.
