Provides Nurse-Family Partnership free maternal and child health services in Chemung County for first-time mothers. Clients receive health care guidance and education during pregnancy and after the birth of the baby until the child’s second birthday, to ensure healthy outcomes for both mother and child.

Provides clinical services to individuals with disabilities and their families including treatment coordination during the eligibility process and while accessing additional agency services; advocacy with other agencies when additional services are needed; psychological services including assessments and therapy; social work; nursing services; nutrition services and rehabilitation counseling. Serves as a one-stop source of information, activities and materials to help improve parent-child interaction through parenting workshops, developmental and health screenings, supervised visits, a comprehensive library and home visits when needed.

Acts in a supporting role to DD State Operations Offices with regard to eligibility for others areas of statewide services including but not limited to: level of care determinations; clinical delivery and waiver service delivery; Article 16 clinics; quality improvement processes; review of audit reports for trend analysis; emergency preparedness; safety, security and maintenance; and implementation of OPWDD initiatives.

Offers a voluntary developmental evaluation and services program offered by New York State and administered locally by the Nassau County Department of Health Office for Children with Special Needs. To be eligible for EI services, children must be under 3 years of age and have a confirmed disability or established developmental delay, as defined by New York State, in one or more of the following areas of development: physical, cognitive, communication, social-emotional, and/or adaptive. Evaluations are conducted to determine eligibility. A variety of therapeutic and support services are offered to eligible infants and toddlers with disabilities and their families. Major provisions of the program include: Identification and referral of children at risk or suspected of disability by primary referral sources; Periodic developmental screening and tracking of at risk children; Service coordination for eligible children and families; A multidisciplinary evaluation provided at no cost to parents to determine eligibility; Individualized Family Service Plans (IFSPs) for eligible children and families; Provision of early intervention services in the IFSP at no cost to parents and Delivery of services in natural settings in the community where children of the same age are typically found, to the maximum extent appropriate. Provides case management and educational advocacy support for children with disabilities and their families. Provides Pre-Employment Transition Services to school age youth between the ages of 14-21. Provides screenings, assessments, ancillary services (PT, OT, speech therapy), respite, and health and mental health clinic services.

A home visiting program for Chautauqua County mothers and their children from pregnancy to age 2. Helps families to achieve healthy outcomes through an evidence-based home visiting curriculum, developmental screenings for children, screenings for mothers, and resources and referrals to address social determinants of health. Provides the following services: - Education on health topics and private supportive visits in the comfort of the family's home - Linkage with nutrition programs and breastfeeding supports - Assistance finding a primary care doctor, dentist, physicians who can address any current health concerns, and medical/health insurance - Assistance applying for WIC - Assistance obtaining family planning and birth control - Linkage to other services in the community - Assistance scheduling appointments for mothers and children - Assistance with transportation needs and translation - Advocacy

Provides a home-based program for all new and expectant parents in order to identify family strengths and provide support during the parenting journey. The program provides the newest child developmental information, popular parenting trends and connects parents to resources.

The Center offers multi-disciplinary assessments and evaluations for children suspected of having a developmental disability, including autism spectrum disorder. If the results of the assessment indicate the need for treatment, the team will provide individualized behavioral programming, occupational therapy, speech therapy and medical management including developmental pharmacology. Also specializes in the treatment of children, adolescents, and adults with behavioral, emotional, and psychiatric difficulties.

Provides the following Clinical Services for children, adolescents and adults in person or via telehealth: Counseling, Anger Management, Risk Assessments, Guardianship, Autism Testing, Occupational Therapy, Physical Therapy, Speech Therapy, Feeding/Swallowing Therapy, Psychiatric Medication Services, Vocational Rehabilitation Counseling, Wheelchair Seating/Positioning, Orientation and Mobility Training, and Psychological Evaluations. Provides the following Clinical Services: Social Work, Occupational Therapy, Physical Therapy, Speech Language Pathology, Psychiatry, Rehabilitation Counseling, and Psychology/Evaluation/Diagnosis.

Provides home- and community-based therapeutic and educational services along with multi-disciplinary evaluations for children ages birth - 5 years old with suspected or identified developmental delays. Services include occupational therapy, physical therapy, speech/language therapy, and special education. Performs screenings of preschool-aged children. Also provides a screening and assessment program for infants in order to detect and prevent developmental problems. Refers the children for appropriate remediation and evaluation.

Provides home visiting services for pregnant women and new parents. The program provides prenatal information to support a healthy pregnancy and support new parents by offering education and information to encourage healthy child development.
