Offers a voluntary developmental evaluation and services program offered by New York State and administered locally by the Nassau County Department of Health Office for Children with Special Needs. To be eligible for EI services, children must be under 3 years of age and have a confirmed disability or established developmental delay, as defined by New York State, in one or more of the following areas of development: physical, cognitive, communication, social-emotional, and/or adaptive. Evaluations are conducted to determine eligibility. A variety of therapeutic and support services are offered to eligible infants and toddlers with disabilities and their families. Major provisions of the program include: Identification and referral of children at risk or suspected of disability by primary referral sources; Periodic developmental screening and tracking of at risk children; Service coordination for eligible children and families; A multidisciplinary evaluation provided at no cost to parents to determine eligibility; Individualized Family Service Plans (IFSPs) for eligible children and families; Provision of early intervention services in the IFSP at no cost to parents and Delivery of services in natural settings in the community where children of the same age are typically found, to the maximum extent appropriate. A home visiting program for Chautauqua County mothers and their children from pregnancy to age 2. Helps families to achieve healthy outcomes through an evidence-based home visiting curriculum, developmental screenings for children, screenings for mothers, and resources and referrals to address social determinants of health. Provides the following services: - Education on health topics and private supportive visits in the comfort of the family's home - Linkage with nutrition programs and breastfeeding supports - Assistance finding a primary care doctor, dentist, physicians who can address any current health concerns, and medical/health insurance - Assistance applying for WIC - Assistance obtaining family planning and birth control - Linkage to other services in the community - Assistance scheduling appointments for mothers and children - Assistance with transportation needs and translation - Advocacy

A preventive health program which provides medical exams and follow-up care care to children and teens under 21 years of age to ensure that they are healthy and growing properly. Makes use of a full range of healthcare services to detect health problems before they become serious and hard to treat. Depending on the age of a child, a Child / Teen Health Plus screening will include: - Blood Tests (such as, blood lead level, sickle cell anemia, etc.) - Complete Physical - Dental Screenings - Developmental Assessment - Health History - Hearing and Vision Testing - Immunizations The child's medical provider will arrange for followup treatment of problems uncovered during the examination.

Provides home- and community-based therapeutic and educational services along with multi-disciplinary evaluations for children ages 0-5 with developmental delays.
