A home visiting program for Chautauqua County mothers and their children from pregnancy to age 2. Helps families to achieve healthy outcomes through an evidence-based home visiting curriculum, developmental screenings for children, screenings for mothers, and resources and referrals to address social determinants of health. Provides the following services: - Education on health topics and private supportive visits in the comfort of the family's home - Linkage with nutrition programs and breastfeeding supports - Assistance finding a primary care doctor, dentist, physicians who can address any current health concerns, and medical/health insurance - Assistance applying for WIC - Assistance obtaining family planning and birth control - Linkage to other services in the community - Assistance scheduling appointments for mothers and children - Assistance with transportation needs and translation - Advocacy
