Provides specialized medical care in all major fields of medicine.

Offers elderly or severely disabled individuals residing at home a day program providing nursing and medical care, case/care management and recreation. Services include access to a variety of medical specialists (dentist, podiatrist, audiologist, therapists), and individualized nursing care for a wide range of medical conditions associated with chronic illness and aging including Alzheimer's, dementia, diabetes and Parkinson's Disease. Program attendance is based on individual needs. Transportation to and from the Center is available.

GMT Team has the capacity to travel and visit clients in their own homes, adult care or nursing home for the purpose of making assessments, monitoring the client's response to treatments and providing emotional support to the clients and/or family members.

• Incontinence
• Increasing memory loss, confusion or disorientation
• Multiple chronic illnesses, especially if there are multiple medication and/or doctors

• Increasing problems with self-care, and living independently
• General frailty

Provides medication for Alzheimer's Disease and other cognitive disorders group and/or individual therapy medication for psychiatric and behavioral disturbances psychological treatment approaches caregiver psychoeducation and support referral to other relevant community resources opportunities to participate in ongoing research on medications to improve memory. Geriatric care managers conduct home assessments for aging-in place, assist families in completing Medicaid applications, facilitate estate planning and geriatric care management. Provide elderly community members and their families with information on aging and resources available.

Assists in the selection of homecare services, sets up individual care plans, provides education and support for patients and caregivers. Also provides Care Aides for home care.
