New York State and Nassau County funding may provide up to 20 hours of aide assistance weekly. Chore workers can provide a few hours of light housekeeping and shopping for those who do not require hands on care.

Offers elderly or severely disabled individuals residing at home a day program providing nursing and medical care, case/care management and recreation. Services include access to a variety of medical specialists (dentist, podiatrist, audiologist, therapists), and individualized nursing care for a wide range of medical conditions associated with chronic illness and aging including Alzheimer's, dementia, diabetes and Parkinson's Disease. Program attendance is based on individual needs. Transportation to and from the Center is available.

In coordination with Saint Francis Hospital, the Saint Francis Hospital Outreach Bus provides free transportation for services: Nursing Services include blood pressure screening and monitoring of other chronic conditions, health information and/or education, in-home assessment, medication management counseling and information, referral to home healthcare services, healthcare coordination, fall prevention screenings and education, and brain games. Social Work Services include individual, family and group counseling, In-home assessment, case management, bereavement support, caregiver assistance, advance directive education, entitlements/Benefits assessments and assistance (food stamps, home heating, low-income programs), disability resource information, financial counseling (referrals), food and nutrition support (Meals on Wheels, Food Pantries, referrals)Medicare, Medicaid or other health insurance information, mental health referrals, respite care (referrals), transition assistance from hospital to home or rehabilitation, home maintenance referrals, support groups, Veterans assistance, Circle of Support (Life planning tool).
Case management for seniors with behavioral health needs. A team including an RN, case counselor and peer will provide confidential, comprehensive in-home assessment, develop a care plan and provide intensive short-term case management for seniors 55+ struggling with behavioral health, substance use or cognitive impairments that interfere with accessing community services and resources. Barriers to accessing services will be addressed.
