  • Falling, balance, and mobility problems
  • Dizziness
  • Incontinence
  • Increasing memory loss, confusion or disorientation
  • Multiple chronic illnesses, especially if there are multiple medication and/or doctors
  • Persistent problems with mood, motivation, or sleeping
  • Chronic pain or discomfort, including arthritis
  • Increasing problems with self-care, and living independently
  • General frailty
* Offers the following services:
  • Complete evaluations of the medical, functional, cognitive and psychosocial problems, and abilities of the older adult
  • Help in coping with the health problems and social needs associated with aging
  • Ongoing medical follow-up and case management for patients with dementia
  • Education, counseling and guidance for families, caregivers, and other professionals
  • Collaboration with the primary care physician and other health care providers
