AUDIOLOGICAL EVALUATIONS AND CONSULTATION SERVICES | DEPARTMENT OF SPECIAL EDUCATION

Provides a full complement of audiological evaluation and consultation services that is available to support the education of students with hearing loss, either in-district or in an Eastern Suffolk BOCES setting. In addition to full audiological evaluations, follow-up, parent and staff training, Eastern Suffolk BOCES offers a full spectrum of services to support the use of Frequency Modulation (FM) auditory trainers, including needs assessment evaluations, selection and ordering, equipment adjustments, summer maintenance, ear molds, training workshops, and review of district equipment.

EARLY INTERVENTION PROGRAM | NASSAU COUNTY DEPARTMENT OF HEALTH

Offers a voluntary developmental evaluation and services program offered by New York State and administered locally by the Nassau County Department of Health Office for Children with Special Needs. To be eligible for EI services, children must be under 3 years of age and have a confirmed disability or established developmental delay, as defined by New York State, in one or more of the following areas of development: physical, cognitive, communication, social-emotional, and/or adaptive. Evaluations are conducted to determine eligibility. A variety of therapeutic and support services are offered to eligible infants and toddlers with disabilities and their families. Major provisions of the program include: Identification and referral of children at risk or suspected of disability by primary referral sources; Periodic developmental screening and tracking of at risk children; Service coordination for eligible children and families; A multidisciplinary evaluation provided at no cost to parents to determine eligibility; Individualized Family Service Plans (IFSPs) for eligible children and families; Provision of early intervention services in the IFSP at no cost to parents and Delivery of services in natural settings in the community where children of the same age are typically found, to the maximum extent appropriate. VISION SERVICES | CENTER FOR COMMUNITY ADJUSTMENT

Offers Vision Services, including Assistive Technology and Orientation and Mobility Assessment and Training, for students attending district programs. Vision Assistive Technology Evaluations identify assistive technology devices and services that will help the student access the general curriculum and achieve academic success. Some of the technologies include screen reading software, speech output, scan-and-read systems, computer screen magnification, electronic note takers, and magnifying Closed-Circuit Televisions (CCTVs). Orientation and Mobility (O&M) is an important and integral part of the curriculum in the comprehensive delivery of services to children with visual impairments. Orientation is the process of using sensory information to establish and maintain one's position in the environment; mobility is the process of moving safely, efficiently, and gracefully within one's environment. The ultimate goal of O&M instruction is for visually impaired persons to be able to travel in any environment as independently as possible. PRESCHOOL PROGRAM - DIX HILLS | DIX HILLS SITE

Provides therapeutic preschool, nursery school, and therapeutic services to children ages 2.5 to 5 years. Children with diagnosed disabilities and developmental delays are educated with typically developing nursery school peers. Provide evaluation and services in the areas of special education, speech therapy, occupational therapy, physical therapy, music therapy, vision therapy, counseling, social work services and psychological services. Consent for evaluation required through school district of residence. Infants and toddlers from birth to 3 years receive in-home evaluations. If eligible, services are provided in natural environments (home/community/day care facilities) through the Early Intervention Program as authorized by Suffolk County Division of Services for Children with Special Needs.

PRESCHOOL SPECIAL EDUCATION PROGRAM | NASSAU COUNTY DEPARTMENT OF HEALTH

Provides services to children who are 3 to 5 years old with developmental delays and/or educational needs. Eligibility is based on the results of a multi-disciplinary evaluation that is at no direct cost to the family. The local school district's Committee on Preschool Special Education (CPSE) determines eligibility, recommends appropriate special education and/or related services and creates an Individualized Education Program (IEP) for each eligible child. The New York State Education Department (NYSED) administers, through local school districts, the Preschool Special Education Programs for preschool students with disabilities, 3 to 5 years of age. The Nassau County Department of Health contracts with providers and pays for these services along with NYSED.
