Provides coordinated health care and support services to the disabled and chronically ill in their homes. Nurses monitor patient and supervise aides in patient's home bimonthly. Social workers evaluate patients and obtain needed services. Through the doctor the patient can receive needed therapies such as speech, physical, occupational, respiratory and nutrition therapy.
