Aids individuals with intellectual and developmental disabilities in making choices and finding individualized services. They provide help in navigating through available services including assistance with eligibility determination, service coordination, and advocacy services. Service coordination is also available to children ages birth to 3 years old who have been diagnosed with a developmental disability or delay.

Provides information and referral for family caregivers to find answers to general questions or information on specific disability and health care needs. Also provides referrals for direct services such as paying for health care, equipment or transportation.

Provides integrated, supportive housing for people with disabilities who are facing homelessness. Apartments are available for Medicaid recipients 55 and older who require assistance with activities of daily living; individuals who are 18 and older with eligibility for services through the NYS Office for People with Developmental Disabilities (OPWDD); and military veterans with disabilities. Staff is located on site who advocate for residents; connect them to services available in the community; provide independent living skills training, and facilitate social and recreational opportunities.

Offers various recreational and socialization services, outreach programs as well as peer support and role model programs.

Provides assistance to individuals with developmental disabilities in gaining access to the necessary support and aid needed to live productive lives. Uses person-centered planning focusing on the needs and desires of the individual receiving services while striving for the highest quality of life. Develops an Individualized Service Plan (ISP) which outlines what goals the individual hopes to accomplish, the strategies to achieve these goals, and what support they will receive along the way. Plans are evaluated and updated as the individual progresses and to ensure all support and services are meeting the individual's needs. Also assists individuals with services such as future planning, residential planning, recreation, residential habilitation, day habilitation, reimbursement, adaptive equipment, and environmental modifications.

Provides information and referral, independent living skills instruction, peer support services, advocacy, and community integration services to individuals with disabilities in order to empower them to live within the community and maintain independence.

Provides consumer-driven services to support individuals with disabilities and their families in achieving their goals by offering information and resources, advocacy, service coordination, various workshops, family reimbursement opportunities, and connection with other individuals or with disabilities.

Provides assistance in gaining and appealing SSDI, SSI, as well as administers Pooled Trust accounts.

Protects the rights of employees and employers by ensuring the proper delivery of benefits to those who are injured or ill, and by promoting compliance with the law. Call for more information.

A consumer driven personal assistance program that allows people with disabilities to gain more control over their personal assistance services. CDPAP enables self directing consumers to advertise, recruit, interview, hire, supervise, train and dismiss their own personal assistant(s). Self directing persons assess their own needs, determine how and by whom these needs should be met, and monitor the quality of services received. Individuals independently make all decisions and manage services directly. If an individual is unable to do so, they may use a certain family member or friend to manage needed services. Community based service group which lends sick room equipment such as wheelchairs, walkers, crutches, bath seats, commodes, bedside tables, etc, to residents of the Three Village School District. Contact is made by phone and answering machine.

Educates SSI and SSDI beneficiaries and their families about federal, state, and local work incentives and benefits.

Provides behavioral and educational consultation and support to individuals, families, and professionals based upon them principles of Applied Behavior Analysis (ABA). Our consultation services and trainings are designed to teach those who support individuals with autism and other special needs (school and agency personnel, family members/caregivers, community members) by providing effective education, training, and support within the frameworks of Applied Behavior Analysis (ABA) and Positive Behavior Interventions and Supports (PBIS). Our Autism Supportive Environment program provides training and assistance to businesses and community organizations in designing environments which are more supportive for individuals with autism and other special needs and their families/caregivers.

Provides residential living in supervised group homes or supportive apartments for intellectual disabilities adults ages 18 and over.

Serves children ages 5-21 with a diagnosis of autism, intellectual disability, or multiple disabilities. Teaches language and communication skills, self-help and activities of daily living, and behavior management. Links individuals to a wide array of long-term supports and services to help them live independently. Specialists provide information on public benefits, home care, accessible transportation, assistive equipment and technology, caregiver supports, home-delivered meal programs, health insurance, respite and more.

Provides a 32 bed care facility for seniors and adults in need of supervision. Services include personal care, supervised medication, meals, laundry and housekeeping services, transportation and social events.

Offers quarterly meetings for parents and professionals to answer critical questions faced by young adults as they prepare to leave their educational placements, transitioning from student services to adult services. A Subregional Library of the National Library Service for the Blind and Physically Handicapped. Lends books and magazines that have been professionally recorded onto cassettes to eligible individuals/organizations, along with special playback equipment, catalogs, and related library services. Maintains a Reading Equipment, Aids and Devices (READ) Center which displays adaptive reading equipment, by appointment only. Serves residents of Long Island of all ages who have a visual or physical disability. Offered by public libraries, in person at agency, or postage-free mail.

Provides community residence programs for adults with a psychiatric disability over 18 yrs. of age with varying levels of supervision and assistance. Offers residential treatment for substance abuse. Licensed by the New York State Office of Alcohol and Substance Abuse Services (NYSOASAS).

Provides adult residential services to developmentally disabled individuals, ages 21 and over who exhibit challenging behaviors. These homes are located at multiple locations in Suffolk and Nassau Counties.

Advises governor, legislature, and state agencies on issues facing persons with disabilities. Initiates and shapes innovative programs and identifies areas for improvement. Fosters coordination through systems advocacy and facilitates community development activities. Provides technical assistance on barrier-free design, legal rights,assistive technology to local governments, providers, families, and maintains toll-free information and referral service.

Volunteers provide home delivered meals for the homebound, handicapped, chronically ill or convalescent persons in the community who are unable to prepare meals in Northern Brookhaven. Food prepared at St. Charles and Mather Hospitals and Port Jefferson Healthcare Facilities.
