Aids individuals with intellectual and developmental disabilities in making choices and finding individualized services. They provide help in navigating through available services including assistance with eligibility determination, service coordination, and advocacy services. Service coordination is also available to children ages birth to 3 years old who have been diagnosed with a developmental disability or delay.

Provides information and referral for family caregivers to find answers to general questions or information on specific disability and health care needs. Also provides referrals for direct services such as paying for health care, equipment or transportation.
Provides integrated, supportive housing for people with disabilities who are facing homelessness. Apartments are available for Medicaid recipients 55 and older who require assistance with activities of daily living; individuals who are 18 and older with eligibility for services through the NYS Office for People with Developmental Disabilities (OPWDD); and military veterans with disabilities. Staff is located on site who advocate for residents; connect them to services available in the community; provide independent living skills training, and facilitate social and recreational opportunities.

Drop In Centers
Provides assistance to individuals with developmental disabilities in gaining access to the necessary support and aid needed to live productive lives. Uses person-centered planning focusing on the needs and desires of the individual receiving services while striving for the highest quality of life. Develops an Individualized Service Plan (ISP) which outlines what goals the individual hopes to accomplish, the strategies to achieve these goals, and what support they will receive along the way. Plans are evaluated and updated as the individual progresses and to ensure all support and services are meeting the individual's needs. Also assists individuals with services such as future planning, residential planning, recreation, residential habilitation, day habilitation, reimbursement, adaptive equipment, and environmental modifications.

A consumer driven personal assistance program that allows people with disabilities to gain more control over their personal assistance services. CDPAP enables self directing consumers to advertise, recruit, interview, hire, supervise, train and dismiss their own personal assistant(s). Self directing persons assess their own needs, determine how and by whom these needs should be met, and monitor the quality of services received. Individuals independently make all decisions and manage services directly. If an individual is unable to do so, they may use a certain family member or friend to manage needed services. Provides behavioral and educational consultation and support to individuals, families, and professionals based upon them principles of Applied Behavior Analysis (ABA). Our consultation services and trainings are designed to teach those who support individuals with autism and other special needs (school and agency personnel, family members/caregivers, community members) by providing effective education, training, and support within the frameworks of Applied Behavior Analysis (ABA) and Positive Behavior Interventions and Supports (PBIS). Our Autism Supportive Environment program provides training and assistance to businesses and community organizations in designing environments which are more supportive for individuals with autism and other special needs and their families/caregivers.

A Subregional Library of the National Library Service for the Blind and Physically Handicapped. Lends books and magazines that have been professionally recorded onto cassettes to eligible individuals/organizations, along with special playback equipment, catalogs, and related library services. Maintains a Reading Equipment, Aids and Devices (READ) Center which displays adaptive reading equipment, by appointment only. Serves residents of Long Island of all ages who have a visual or physical disability. Offered by public libraries, in person at agency, or postage-free mail.
