Assists with pursuing OPWDD eligibility status determination, self-advocacy, and person center planning, provides funding assistance for disability related services and supplies and recreation opportunities within the community for disability enrolled individuals and family members.

Uses person-centered planning focusing on the needs and desires of the individual receiving services while striving for the highest quality of life. Provides consumer-driven services to support individuals with disabilities and their families in achieving their goals by offering information and resources, advocacy, service coordination, various workshops, family reimbursement opportunities, and connection with other individuals or with disabilities.

Offers various recreational and socialization services, outreach programs as well as peer support and role model programs.

Serves children ages 5-21 with a diagnosis of autism, intellectual disability, or multiple disabilities. Provides coordination of services for legally blind veterans and their families. Duties include providing and/or arranging for the provision of appropriate treatment modalities to enhance a blinded veteran's functioning level. Other duties include identifying new cases of blindness, providing professional counseling, resolving problems, arranging annual healthcare reviews and conducting education programs relating to blindness.

Provides free curb-to-curb transportation within the township for residents, whose disability restricts their mobility and prevents them from using traditional forms of transportation.
