Provides individuals who suffered a neurological injury/illness and their family members information,education and support to assist in coping with the challenges that arise as a result of their illness and promote wellness. Provides crisis prevention and response services to people with intellectual/developmental disabilities or complex behavioral/mental health needs, their families and caregivers with the goal of enabling individuals to remain in their home or community. Offers needs assessment develops and individualized crisis plan, recommends preventive and coping strategies for families/caregivers and provides referrals to appropriate agencies. Provides short-term therapeutic in-home supports, therapeutic emergency and planned Resource Center services for enrolled individuals. Provides consultation, training and technical assistance to community partners and organizations.
