Provides financial assistance to needy kidney patients, transplant recipients, and donors to help cover the cost of treatment, medication transportation, emergency transient dialysis, medical equipment, etc. Has several programs: Individual Grants Program, Pharmacy Grant Program, Emergency Fund Program, Discount Pharmacy Card, Health Insurance Premium Program, Disaster Relief Program, Sherer Travel-Related Dialysis Program, and the Pediatric Campership Program.

Provides preventive information to reduce the high risk of kidney disease among minorities. Also produces a brochure series for African-Americans and Hispanic populations, as well as Spanish-language brochures.

Helps dialysis patients get back on their feet when catastrophic events strike their communities. Provides funds to assist patients, replace medications, food and household items and pay for other necessities lost because of natural disasters.

Provides renal care for adult patients with end-stage renal disease including in-center hemodialysis, end-stage renal disease exercise counseling program, educational programs, and access to transplant programs. Offers vacation and seasonal scheduling.

Raises awareness about Polycystic Kidney Disease (PKD) and the PKD Foundation on a local level. In addition to offering support and information, promotes ways to raise funding to advance research and education. Provides financial assistance to kidney patients and education to those with and at risk for kidney disease. Programs address all stages of kidney health, including kidney disease awareness and prevention community outreach to individuals at high risk for kidney disease and treatment related support for those requiring renal dialysis and transplantation. Also advocates for patients' access to health information and education to quality care, including treatment choices and to affordable medication and health insurance coverage.
