Classes are offered at various locations throughout the county, during day and evening, to promote self-management of diabetes and weight management in efforts to prevent or delay the complications associated with diabetes.

Offers elderly or severely disabled individuals residing at home a day program providing nursing and medical care, case/care management and recreation. Services include access to a variety of medical specialists (dentist, podiatrist, audiologist, therapists), and individualized nursing care for a wide range of medical conditions associated with chronic illness and aging including Alzheimer's, dementia, diabetes and Parkinson's Disease. Program attendance is based on individual needs. Transportation to and from the Center is available.
