Offers Nassau and Suffolk County residents with intellectual and/or developmental disabilities and their families the ability to select services and providers that are most suited to their needs. Offers self-directed community habilitation, respite and SEMPT (supported employment). Assisted by a Fiscal Intermediary and Support Broker, individuals and family members learn how to implement HCBS (Home & Community Based) waiver services establish a habilitation plan, assess needs, and budget allocated Medicaid funds.

Provides support to individuals, 18 years of age and older with developmental disabilities transitioning from school, in developing skills for independent living and employment. Offers health and wellness groups, relaxation techniques, speech and physical therapy. Provides assistance in community integration. Provides evaluations for individuals with a developmental disability for determination of competitive or supported employment to include job training.

Provides residential care, respite and service coordination for adults and children. Offers foster residential placement for developmentally disabled adults.

Provides 24 hour residential services, day habilitative and day treatment program services for developmentally disabled adults, 18 years of age and older.

Provides residential living in supportive apartments for intellectually disabled adults.

Catholic Charities operates group homes for adults with intellectual and developmental disabilities that encourage each resident to grow in their functioning and to participate in their communities. As able, residents participate in day programs and workshops, engage in social and recreational activities in the community, have household chores, and run errands. Some even maintain competitive employment. Residential services are provided in 13 residences serving 107 people in communities throughout Long Island including: Amityville (two residences, one for developmentally disabled adults with Alzheimer's disease/dementia) Aquebogue, Babylon Village, Copiague, Elmont, Hampton Bays, Huntington Station, Manhasset (for Developmentally disabled deaf adults) Southhold, Valley Stream, West Babylon (two residences)

Serves children ages 5-21 with a diagnosis of autism, intellectual disability, or multiple disabilities. Provides a religious school for Jewish children with special needs. Also offers cultural enrichment, parent support groups and socialization groups.

Provides Intermediate Care Facilities (ICF) for the mentally developmentally disabled, in a family homelike atmosphere in a community setting. The population of these facilities range in age and profound to moderate in functioning level. Both males and females. Facilities throughout New York City and Long Island.

Provides and opportunity for seniors to serve as companions to developmentally disabled adults ages 21 and older. Offers a small stipend for low income seniors who act as role models and advocates Seniors must have a medical release, be free from critical illness, have a tuberculin test and must be low income.

Offers quarterly meetings for parents and professionals to answer critical questions faced by young adults as they prepare to leave their educational placements, transitioning from student services to adult services. Provides a toy lending and resource center for families of children (birth-15 years of age) with disabilities. Provides an extensive library of toys (conventional and adapted), therapeutic equipment and books for parents and children to borrow for one month.

Provides year-round athletic training and competition for children and adults with intellectual disabilities. Competition takes place throughout the county in over 20 different olympic-type sports
