A 24-hour state operated supervised residential care center for adults. Offers support skills training to individuals with a diagnosis of mental illness who are preparing for more independent living environments.

Offers two certified mental health community residence for adults who need assistance preparing for successful community living.

Offers several community based residential and supported housing programs that address the specific needs of clients. Supportive housing funds are targeted to three populations: Those leaving Psychiatric centers. Those determined to be eligible through the diagnosis and assessment from a licensed mental health clinician. Individuals who are currently homeless.

* Provides habilitation services and treatment for children which results in outcomes of successful functioning and intergration back into community, family and school setttings.

Provides short-term crisis support in order to avert an unnecessary psychiatric hospitalization, prevent a lengthy hospitalization, and/or provide step down support following hospitalization prior to return to independent living status or while awaiting alternative community placement.

Provides a safe space to support individuals in all stages of recovery. Offers various programs, meetings, and special events that are inclusive to the entire recovery community. The purpose is to help the individual and their support network develop the skills they require to maintain a successful recovery. This is a drop-in center and individuals are welcome and encouraged to drop in at any time during hours of operation.

Support for youth with mental health challenges through: Whole Child Checkup Initiative: encourages brief mental health screenings as part of regular check ups with primary care doctors for early identification of mental health issues and connect families with appropriate resources Solutions for Youth and Families Warm Line: helps individuals find appropriate mental resources in the community Collaborative Care Community Model: "best practices" for youth transitioning to or from hospitals or residential care to decrease setbacks and days missed from school

CTRC houses an adult outpatient clinic which offers comprehensive mental health services to individuals with qualifying DSM-IV diagnosis. Self-referrals accepted.

Certified Peer Staff facilitate access to services, supports, opportunities and help others navigate the community in group and individual settings. Daily activities includes arts and crafts, speakers, and classes in a positive setting. Educational and group support for the community.

Assists individuals with functional mental health-related disabilities to successfully identify, pursue, and achieve their life goals at home, work, school, and in the community. Program is provided in collaboration with Capabilities and is A NYS OMH-licensed comprehensive PROS Program. Clients receive Community Rehabilitation and Support (CRS) services; Intensive Rehabilitation (IR) services; Ongoing Rehabilitation Support (ORS) services; and Clinical Treatment (CT).

Provides supported housing opportunities for individuals recovering from mental illness needing a stable living environment. Individuals are provided the necessary support to live independently. Psychiatric Aftercare Clinic for clients who have previously been inpatient.

Provides brief, intermediate and lifelong living arrangements in a sheltered environment for individuals with acute or chronic mental or emotional disorders.

Provides supervised living programs for 32 residents on the grounds of Pilgrim Psychiatric Center and 44 residents in four houses in the community. Vocational training, volunteer placement, employment, day treatment or continuing education are encouraged. Emphasis is on ADL and enhancing socialization skills. Specialized programs are also available for MICA, and low motivated residents.

Provides 24 hour supervision at local community sites for adults with mental illness. Services provided at each site are restorative services, symptom management, community living skills, information and referral, crisis management, medication education and monitoring, and community integration. Staff provides supportive assistance and instruction in daily living skills and use of community resources. There are ten residences located in Nassau and Suffolk County. Transportation provided to local day treatment services.

Offers a socialization and recovery program to Nassau County residents with severe mental illness, ages 18 and older. Provides opportunities in a "club house setting" for individuals to improve socialization skills, physical, emotional and spiritual health and decision making, reduce symptoms, maintain sobriety, and learn technology skills. Provides services for individuals diagnosed with serious and persistent mental illness in the development of their own recovery plan and goals in the area of work, school, living and socialization. Rehabilitation uses a goal-oriented approach to help participants develop the skills and resources they need to succeed in vocational, educational, residential and social environments. Additional services include preparation for employment, maintaining employment and money management.

Provides learning experiences where individuals can develop strengths and skills that contribute to an individualâs employability in the community. Services include teaching concepts such as compliance, attendance, task completion, problem solving, safety, wellness while at work, socialization at work, communication, accepting supervision, personal appearance and appropriate work attire; goal setting, work ethics, and work tolerance.

Provides outpatient mental health services, such as mental health screening, individual, group, child/adolescent counseling, geriatric services, including after care support.
