Provides clinical intervention and support for adults with serious mental illness utilizing a multidisciplinary team to bring services directly to individuals in the community in a setting that best meets their recovery needs. Services include outreach and engagement, crisis assessment and intervention, community linkage, family and caregiver support and education, peer support services, legal interface, health assessment and related health and wellness education, skill building, psychiatric rehabilitation and recovery, and brief therapeutic support.

Youth Enrichment Programs
Provides services to people enrolled in NYS Medicaid who have specific medical and mental health needs, particularly those living with or in recovery from mental health or substance use disorders. Program helps individuals to be more self-sufficient. Services may be provided in member's home or preferred community location. Program offers Home and Community Based Services (HCBS) and Community Oriented Recovery and Empowerment Services (CORE).
