Provides services to people enrolled in NYS Medicaid who have specific medical and mental health needs, particularly those living with or in recovery from mental health or substance use disorders. Program helps individuals to be more self-sufficient. Services may be provided in member's home or preferred community location. Program offers Home and Community Based Services (HCBS) and Community Oriented Recovery and Empowerment Services (CORE).
Provides monthly teen peer support groups, helps teens develop community service and social-recreational skills by planning, developing and attending regional events.
