Provides a centralized intake process for adults and children who have significant behavioral health needs that would benefit from additional supports and services in the community. This includes people with significant behavioral and medical or social risk factors which can be addressed through overall care management. There are three types of SPOAs - Children's, Adult Case/Care Management and Adult Housing. The SPOA process provides for the identification of people most in need of services, and manages service access and utilization.

Provides linkage to service coordination and other services as needed. Individuals may be linked with a care coordinator who will meet with the individual based on their needs.

This service replaces the Targeted Case Management Services. This agency contracts with Adirondack Health Institute (AHI) and provides linkage and supports to assist individuals with their mental and physical health care needs. OMH funded and contract with Health Home and DOH.

All applications for case management, assertive community treatment, and assisted outpatient treatment are processed through the Nassau County Department of Mental Health where eligibility is determined and assignments are made to the appropriate level of care. Provides Children and Family Treatment and Support Services (CFTSS) to children and youth under the age of 21 who are covered by Medicaid and are enrolled in a Medicaid Managed Care plan. Referrals may be made by parents, teachers, doctors, or anyone connected to the youth.

A community-wide advocacy, training, education and awareness program for people with mental illness, members of their families, mental health professionals, and provider agencies with promotes the total equality and inclusion of persons with mental illness in the development of polices, regulations, and services that affect their lives.

Provides an array of mental health services, such as mental evaluation and assessment and coordinates the mental health needs of the diagnosed individual, along with the daily living skills. Also available is smoking cessation for those who want to stop smoking. Social workers help patients and their families deal with various issues dealing with age, chronic illness or disability.

Manages psychiatric patients in crisis and arranges emergency admissions, when needed, through South Shore Emergency Department. Offers specialized care by psychiatric nurses and psychiatrist consultations/assessments around the clock. Participates in county-wide coordinated and regional system of psychiatric emergency services.

Behavioral Health Services North provides adult, child and family clinical services at our Center for Well-Being – a community mental health clinic.

Offers evaluation and counseling for children, adults, and families struggling with emotional issues. Also provides on-site sexual abuse counseling.

Provides a mental health assessment and links them to treatment, for juvenile delinquents and their families, in an effort to avert out-of-home placement.

Provides mental health care facilities, behavioral health, crisis intervention and evaluations to help those struggling with emotional or mental illness.

The Child Single Point of Access (C-SPOA) is a process that assists children and youth, 0-21, who are struggling emotionally and/or have a mental health diagnosis and their families to access many in-home and community-based services and supports to prevent hospitalizations and out of home placements.

Gatekeeper assists and engages older adults, 55+, who live independently and may be at risk of mental health issues, including depression and anxiety and would benefit from case management services.

Advanced Life Support and Basic Life Support, Ambulance Transport Services staffed by NYS Certified Emergency Medical Technicians and Paramedics. Operational 24 hours a day 7 days a week.

Welcome Orange Geriatric Initiative (WOGI) Gatekeeper Program is a senior mobile outreach program designed to proactively identify at-risk older adults in the community who are not connected to the service delivery system. Assist seniors with services by evaluating their signs and symptoms by engaging and initiating in-home assessments to provide or access a variety of supportive services. This program is designed to keep at-risk seniors in their own homes, and prevent premature out-of-home placement by addressing unmet needs. The program should not be used for services provided by a licensed outpatient program, or for services provided by another active OMH funded program.

Individual, family and group therapy, medication when appropriate and crisis intervention. Intervention program offered for children up to age 18 in families who have undergone a physical or emotional crisis. Treats children with emotional, social and adjustment problems.

Outpatient psychiatric services available to children, adolescents and their families. Common childhood emotional and behavioral problems, such as Attention Deficit Hyperactivity Disorder, are assessed and treated. Psychological and neuropsychological evaluations, medication, play therapy, and parent counseling for enrolled clinic patients are among the services available.

Offers individual, group, couples and family therapy for children, adolescents and adults in NYS licensed mental health clinics. Psychiatric consultation, medication therapy and specialized services for children and trauma survivors are also available. Offers a wide range of services for youth with psychiatric hospitalization and families experiencing problems which interfere with healthy development of their children. Includes treatment for youth through age 21 at risk of suicide, those adjusting to divorce, or other family crisis, eating disorders, social difficulties, unmanageable behavior, depression, or anxiety disorders. Walk-in services are also available for anyone seeking support.

A committee designed to link families who may require more intensive mental health support to services such as Care Coordination, Home-Based Crisis Intervention, Mental Health Respite Program, Home and Community Based Services, Family Support, and OMH services such as Residential Treatment Facility, Community Residence, Children's Psychiatric Center, or mobile crisis programs. Families are encouraged to invite schools and others working with their child to attend a meeting; approach is strength-based and constructive; jointly crafted written plans are provided to families at the conclusion of the meeting. Committee is made up of representatives from the following organizations: Wyoming County Department of Mental Health, Social Services, Probation, and Youth Bureau; Spectrum Health & Human Services (Certified Community Behavioral Health Clinic Services including Mental Health Counseling and Chemical Dependency Services), Hillside Children's Center, Wyoming County Community Hospital (Crisis Outreach), GLOW Family Support, Clarity Wellness Community, New Directions and Western New York Children's Psychiatric Center.

Provides outpatient behavioral health services to adults 18 years and older. Serves children under 18 ONLY if they are experiencing first episode psychosis. Treatment types include individual and group therapy. Services are provided by psychiatrists, nurses, and licensed clinical staff. Offers the NAVIGATE Program, a specialized evidence-based program for individuals experiencing first episode psychosis. Also offers a behavioral health walk-in clinic (The Help Center) that does not require an appointment. The Help Center offers immediate support for individuals who may be experiencing a crisis and/or are in need of behavioral health services and linkage to a community provider. The Help Center is an alternative to ECMC's Comprehensive Psychiatric Emergency Program (CPEP) for those who are NOT experiencing suicidal thoughts/behaviors.
