Individuals will have access to a Psychiatrist, Nurse, Therapist, Peer Specialist and Education and Employment Specialist when enrolled in OnTrackNY.

Provides a centralized intake process for adults and children who have significant behavioral health including medical and/or social risk factors. There are three types of SPOAs - Children's, Adult Case/Care Management and Adult Housing. The SPOA process provides for the identification of people most in need of services, and manages service access and utilization.
The Child Single Point of Access (C-SPOA) is a process that assists children and youth, 0-21, who are struggling emotionally and/or have a mental health diagnosis and their families to access many in-home and community-based services and supports to prevent hospitalizations and out of home placements.

Includes treatment for youth through age 21 at risk of suicide, those adjusting to divorce, or other family crisis, eating disorders, social difficulties, unmanageable behavior, depression, or anxiety disorders.
