Single Point of Access (SPOA) | Chemung County Children's Integrated Services (CIS)

Provides a centralized intake process for adults and children who have significant behavioral health needs that would benefit from additional supports and services in the community. This includes people with significant behavioral and medical or social risk factors which can be addressed through overall care management. There are three types of SPOAs - Children's, Adult Case/Care Management and Adult Housing. The SPOA process provides for the identification of people most in need of services, and manages service access and utilization.

CARE COORDINATION | MENTAL HEALTH ASSOCIATION IN ESSEX COUNTY - TICONDEROGA

This service replaces the Targeted Case Management Services. This agency contracts with Adirondack Health Institute (AHI) and provides linkage and supports to assist individuals with their mental and physical health care needs. OMH funded and contract with Health Home and DOH.

SINGLE POINT OF ENTRY FOR ADULTS - SPOE | NASSAU COUNTY OFFICE OF MENTAL HEALTH, CHEMICAL DEPENDENCY & DEVELOPMENTAL DISABILITIES SERVICES

All applications for case management, assertive community treatment, and assisted outpatient treatment are processed through the Nassau County Department of Mental Health where eligibility is determined and assignments are made to the appropriate level of care. MENTAL HEALTH HOMECARE PROGRAM | VISITING NURSE SERVICES (VNS) WESTCHESTER

Provides an array of mental health services, such as mental evaluation and assessment and coordinates the mental health needs of the diagnosed individual, along with the daily living skills. Also available is smoking cessation for those who want to stop smoking. Social workers help patients and their families deal with various issues dealing with age, chronic illness or disability.

WELCOME ORANGE GERIATRIC INITIATIVE (WOGI) | ORANGE COUNTY DEPARTMENT OF MENTAL HEALTH

Welcome Orange Geriatric Initiative (WOGI) Gatekeeper Program is a senior mobile outreach program designed to proactively identify at-risk older adults in the community who are not connected to the service delivery system. Assist seniors with services by evaluating their signs and symptoms by engaging and initiating in-home assessments to provide or access a variety of supportive services. This program is designed to keep at-risk seniors in their own homes, and prevent premature out-of-home placement by addressing unmet needs. The program should not be used for services provided by a licensed outpatient program, or for services provided by another active OMH funded program.

Children's Single Point of Access (SPOA) - Wyoming County - Mental Health Department

A committee designed to link families who may require more intensive mental health support to services such as Care Coordination, Home-Based Crisis Intervention, Mental Health Respite Program, Home and Community Based Services, Family Support, and OMH services such as Residential Treatment Facility, Community Residence, Children's Psychiatric Center, or mobile crisis programs. Families are encouraged to invite schools and others working with their child to attend a meeting; approach is strength-based and constructive; jointly crafted written plans are provided to families at the conclusion of the meeting. Committee is made up of representatives from the following organizations: Wyoming County Department of Mental Health, Social Services, Probation, and Youth Bureau; Spectrum Health & Human Services (Certified Community Behavioral Health Clinic Services including Mental Health Counseling and Chemical Dependency Services), Hillside Children's Center, Wyoming County Community Hospital (Crisis Outreach), GLOW Family Support, Clarity Wellness Community, New Directions and Western New York Children's Psychiatric Center.

Adult & Family Clinic - ECMC

Provides outpatient behavioral health services to adults 18 years and older. Serves children under 18 ONLY if they are experiencing first episode psychosis. Treatment types include individual and group therapy. Services are provided by psychiatrists, nurses, and licensed clinical staff. Offers the NAVIGATE Program, a specialized evidence-based program for individuals experiencing first episode psychosis. Also offers a behavioral health walk-in clinic (The Help Center) that does not require an appointment. The Help Center offers immediate support for individuals who may be experiencing a crisis and/or are in need of behavioral health services and linkage to a community provider. The Help Center is an alternative to ECMC's Comprehensive Psychiatric Emergency Program (CPEP) for those who are NOT experiencing suicidal thoughts/behaviors.
