Offers a community based program that provides mental health services in the home or community. Services include counseling, medication management, peer services and case management. Community Residences are home-like programs for youth with mental health needs. CRs offer supervision, structure, care coordination, skill building and behavior support. The goal of a CR is to assist youth in achieving their best level of functioning so that they can be successful in school and at home when they return to their home and community. Residential Treatment Facilities are therapeutic programs that provide inpatient psychiatric treatment to youth with complex mental health needs and support to their families. Welcome Orange Geriatric Initiative (WOGI) Gatekeeper Program is a senior mobile outreach program designed to proactively identify at-risk older adults in the community who are not connected to the service delivery system. Assist seniors with services by evaluating their signs and symptoms by engaging and initiating in-home assessments to provide or access a variety of supportive services. This program is designed to keep at-risk seniors in their own homes, and prevent premature out-of-home placement by addressing unmet needs. The program should not be used for services provided by a licensed outpatient program, or for services provided by another active OMH funded program.
