Provides wrap around services to individuals who have been diagnosed with a severe and persistent mental illness. Assists with service planning and coordination, housing supports, money management, family education, substance abuse treatment, vocational support, mental and physical health treatment, and community referrals to offer support, rehabilitation, and treatment within the community.

Provides linkage to services for adults who are seriously and persistently mentally ill.

Provides recovery-based services through individual, couple, family, and group therapy, along with psychiatric and medication services to adults and children who may be experiencing a wide range of mental health and emotional issues or suffering from some form of mental illness. Community Residences are home-like programs for youth with mental health needs. CRs offer supervision, structure, care coordination, skill building and behavior support. The goal of a CR is to assist youth in achieving their best level of functioning so that they can be successful in school and at home when they return to their home and community. Residential Treatment Facilities are therapeutic programs that provide inpatient psychiatric treatment to youth with complex mental health needs and support to their families. Offers a full continuum of care including inpatient psychiatry, outpatient mental health and chemical dependency day programs and clinics, as well as residential programs for those with mental health issues, intellectual disabilities, and chemical dependence.

A community-wide advocacy, training, education and awareness program for people with mental illness, members of their families, mental health professionals, and provider agencies with promotes the total equality and inclusion of persons with mental illness in the development of polices, regulations, and services that affect their lives.

Provides various mental health services, such as assessment and treatment, individual, family and group therapy, psychiatric evaluation and medication monitoring. Also offer coping skills for depression, anxiety, aging issues, retirement, disability and relationship issues.

Provides evaluation and treatment services, through an intensive, short-term (4-9months) residential program, for children with emotional and behavioral problems, and works with the families to develop comprehensive diagnoses, treatment recommendations and referrals.

Evaluates and treats children in the inpatient unit, aged 5-13 years, for emotional and behavioral problems, using short term stays (1-12 weeks). Discharge planning includes school placement, outpatient follow-up and family support. This is a program for Veterans with PTSD, TBI, and other disabilities whereby they receive peer support from other Veterans trained in military behavioral health.

Welcome Orange Geriatric Initiative (WOGI) Gatekeeper Program is a senior mobile outreach program designed to proactively identify at-risk older adults in the community who are not connected to the service delivery system. Assist seniors with services by evaluating their signs and symptoms by engaging and initiating in-home assessments to provide or access a variety of supportive services. This program is designed to keep at-risk seniors in their own homes, and prevent premature out-of-home placement by addressing unmet needs. The program should not be used for services provided by a licensed outpatient program, or for services provided by another active OMH funded program.

OnTrackNY helps these individuals achieve their goals for school, work and relationships. Individuals will have access to a Psychiatrist, Nurse, Therapist, Peer Specialist and Education and Employment Specialist when enrolled in OnTrackNY.
