Care Coordination/Case Management Services are intended for the high need consumer who has had difficulty maintaining linkages to mental health services and other important supports, such as benefits, housing, advocacy, health care, peer services, or vocational services. SPOA is a uniform referral process which assists the referring party to determine the appropriate level of care coordination services. After the referral is made it is assessed and a determination is made as to the acceptance of the initial referral or referral to another program which may better meet the needs of the individual. Levels of care include: transition case management, care coordination, assertive community treatment, and assisted outpatient treatment.

Welcome Orange Geriatric Initiative (WOGI) Gatekeeper Program is a senior mobile outreach program designed to proactively identify at-risk older adults in the community who are not connected to the service delivery system. Assist seniors with services by evaluating their signs and symptoms by engaging and initiating in-home assessments to provide or access a variety of supportive services. This program is designed to keep at-risk seniors in their own homes, and prevent premature out-of-home placement by addressing unmet needs. The program should not be used for services provided by a licensed outpatient program, or for services provided by another active OMH funded program.
