An innovative outpatient treatment program for adolescents and young adults who may be experiencing mental health issues. OnTrackNY helps these individuals achieve their goals for school, work and relationships. Individuals will have access to a Psychiatrist, Nurse, Therapist, Peer Specialist and Education and Employment Specialist when enrolled in OnTrackNY.

Provides psychiatric evaluation, group therapy, psychopharmocology, as well as full inpatient mental health services.

Provides information and possible referrals based on the situation and needs of each person.

Provides linkage to service coordination and other services as needed. Individuals are linked with a care coordinator who will meet with the individual based on their needs.

Provides outpatient mental health counseling. Counseling available for women who are experiencing postpartum mood disorder or postpartum depression. Treatment for depression, anxiety, post traumatic stress disorder, bipolar, and other mental health disorders is also available. Individual and group counseling offered for individuals with co-occurring disorders. Also provides screening and referral, assessment and treatment planning, and case management services.

All applications for case management, assertive community treatment, and assisted outpatient treatment are processed through the Nassau County Department of Mental Health where eligibility is determined and assignments are made to the appropriate level of care. An outpatient clinic for adults who are experiencing depression, anxiety, family conflicts, emotional or psychiatric illness. The program is offered by Psychiatrists, Social Workers, Clinical Nurse Specialists, and Nurse Practitioner. Provides crisis intervention for psychiatric emergencies in the community, assessment of needed mental health services, linking to mental health services, advocacy, and case follow-up.

Provides mental health counseling and medication management to individuals ages 5 years and older.

Provides a centralized intake process for adults and children who have significant behavioral health including medical and/or social risk factors. There are three types of SPOAs - Children's, Adult Case/Care Management and Adult Housing. The SPOA process provides for the identification of people most in need of services, and manages service access and utilization.

Offers mental health services for children and adults, services include evaluation, counseling, case management, crisis intervention, medication management and referrals for higher level care.

The Child Single Point of Access (C-SPOA) is a process that assists children and youth, 0-21, who are struggling emotionally and/or have a mental health diagnosis and their families to access many in-home and community-based services and supports to prevent hospitalizations and out of home placements.

Gatekeeper assists and engages older adults, 55+, who live independently and may be at risk of mental health issues, including depression and anxiety and would benefit from case management services.

Offers individual, group, couples and family therapy for children, adolescents and adults in NYS licensed mental health clinics. Psychiatric consultation, medication therapy and specialized services for children and trauma survivors are also available. Offers a wide range of services for youth with psychiatric hospitalization and families experiencing problems which interfere with healthy development of their children. Includes treatment for youth through age 21 at risk of suicide, those adjusting to divorce, or other family crisis, eating disorders, social difficulties, unmanageable behavior, depression, or anxiety disorders. Walk-in services are also available for anyone seeking support.

Offers outpatient programs and a wide range of services by mental health professionals. Our staff includes psychiatrists, psychologists, licensed clinical social workers and activities therapists. Patients can join individual, group and family psychotherapy, and we offer specialized services for depression, anxiety and childhood emotional disorders. Specific programs include: Adult Psychotherapy Program for individual or group therapy; Family and Children's Program offering individual, group and family psychotherapy for children and their families and Options for more intensive care at South Nassau: Partial Hospitalization Program, five days weekly; Intensive Outpatient Program, three days weekly and Inpatient Behavioral Health Unit. DBT treatment is quite comprehensive and involves FOUR components: Individual therapy; Group skills training classes; Between-session telephone coaching and Consultation team for therapists. A unique aspect of our treatment package is that we have added a family therapy component to traditional DBT treatment in our clinic. The ultimate goal of dialectical behavior therapy is to help our clients and their families create a life worth living. This means accepting the pain that comes with life and all its struggles while engaging in value-based living. Back and Neck Pain Center-Mental Health Clinic-Outpatient Physical Therapy-Outpatient Speech Language Therapy

Provides an array of mental health services, such as mental evaluation and assessment and coordinates the mental health needs of the diagnosed individual, along with the daily living skills. Also available is smoking cessation for those who want to stop smoking. Social workers help patients and their families deal with various issues dealing with age, chronic illness or disability.

Provides an outpatient/inpatient clinic to help adults and children struggling with mental or emotional problems. Services include evaluation/screening, diagnostic and treatment planning services for people who are experiencing acute or chronic psychiatric problems, individual, group and family therapy, also crisis intervention.

Assists children and their families with a multidisciplinary team to provide support during and after treatment. Provides services to children ages 5 to 18 years, who have problems that range from mild behavior disorders to psychiatric illnesses.
