Provides a court ordered service for the adult clients who are non-compliant with psychiatric medication and as a result decompensate and are deemed unable to live safely in the community. The initial AOT order lasts for six months and if renewed the order could last for another year. Mandated services include, but are not limited to, medication management and case management or ACT. The Nassau County Department of Mental Health is required to maintain oversight responsibility for all clients on AOT orders. Call for additional information and inquiry.

Access is a hotline with trained counselors who can speak with callers going through mental health issues, personal crisis, who need someone to talk to about depression, or are having family difficulties. The Access hotline can dispatch counselors to respond on location to access the crisis, develop a follow up plan, and stay involved to make sure the plan is carried out. Same day counseling sessions are offered in the office with and without appointments.

Consists of licensed professionals (social workers and nurses) specially trained to respond to a wide range of serious and potentially disabling mental health problems. The team is mobile and designed to respond quickly to individuals and/or families in need of crisis intervention at home or anywhere in the community. The team members travel to the home or location in Nassau County to provide on-site, professional and supportive crisis intervention for any child or adult experiencing an emotional disturbance, interpersonal crisis or psychiatric emergency. The Nassau County Mobile Crisis Team is a service provided through the joint efforts of the New York State Office of Mental Health, Nassau County Division of Mental Health, South Shore Child Guidance Center, the Nassau County Police Department and the Emergency Program at Nassau University Medical Center (NUMC). The primary function of the team is to offer an alternative to police intervention or to assist police with the goal of minimizing a potential crisis in the community. Team members assess the problem, evaluate the psychological condition, functional level, environment, and safety of the individual(s) or family in crisis. Referrals are made to the most appropriate treatment provider or facility, using all available resources in the community. In addition, there is follow up on all referrals to assure continuity. In Person Crisis Intervention
This mobile crisis service assists individuals of all ages through mental health and substance abuse crises by providing them with alternative community services. They also provide telephone consultation and information to family/adults experiencing crisis and also professionals who are working with such individuals. Face-to-face crisis assessments is performed after screening of appropriateness. Crisis intervention and stabilization services can be given on-site at the individuals location in the community. Care management follow-up is used specifically to facilitate transition from crisis intervention and stabilization to other agencies in the community for ongoing treatment and support. If individual require additional evaluation for inpatient treatment; referrals to services are available to them, such as outpatient mental health services, emergency department, housing, child services, etc.
Provides a free, confidential, 24 hours mental health service for anyone who is experiencing a mental health crisis. The mobile team can go anywhere in the community to evaluate and provide crisis intervention and referrals to services and agencies as required.

Designed to prevent psychiatric hospitalization, HBCI is an intensive, short-term (4-6 weeks) crisis intervention service and family education program. Therapists are available to families 24 hours/day, 7 days/week, to respond to a crisis situation.

Welcome Orange Geriatric Initiative (WOGI) Gatekeeper Program is a senior mobile outreach program designed to proactively identify at-risk older adults in the community who are not connected to the service delivery system. Assist seniors with services by evaluating their signs and symptoms by engaging and initiating in-home assessments to provide or access a variety of supportive services. This program is designed to keep at-risk seniors in their own homes, and prevent premature out-of-home placement by addressing unmet needs. The program should not be used for services provided by a licensed outpatient program, or for services provided by another active OMH funded program.
