PATHWAYS - SUPERVISED PROGRAM | TRANSITIONAL SERVICES OF NEW YORK FOR LONG ISLAND

Provides supervised living programs for 32 residents on the grounds of Pilgrim Psychiatric Center and 44 residents in four houses in the community. Vocational training, volunteer placement, employment, day treatment or continuing education are encouraged. Emphasis is on ADL and enhancing socialization skills. Specialized programs are also available for MICA, and low motivated residents.

OUTPATIENT CLINIC | CHAMPLAIN VALLEY FAMILY CENTER

Provides a comprehensive outpatient treatment program for individuals struggling with alcohol and drug addiction. Also does public awareness seminars about substance abuse and the resources of help available.

TRANSITIONAL LIVING SUPERVISED RESIDENCE | OSWEGO COUNTY OPPORTUNITIES (OCO)

Provides a 10-bed facility with 24 hour oversight and support for adults with an Axis 1 of severe and persistent mental illness (SPMI) who are unable to live independently. Provides training and assistance in the skills of daily living.

Individuals in residential habilitation services is provided with the support to pursue his/her individualized interests and daily living skills training is offered in such areas as cooking, shopping, money management, laundry care and personal hygiene to maximize a person’s abilities. Active participation in daily life, including the pursuit of community inclusion, recreational, spiritual and social interests, is also fostered.

Staff make routine visits and are available 24 hours a day in case of an emergency. Provides 12 one-bedroom apartments.

Provides 24 hour supervision at local community sites for adults with mental illness. Transportation provided to local day treatment services.

OMH Certified Treatment Apartment Program. A staff person visits these apartments 3-5 times a week, relevant to provision of community rehabilitation skills. Staff are available 24 hours a day, 7 days a week.
