Provides supervised living programs for 32 residents on the grounds of Pilgrim Psychiatric Center and 44 residents in four houses in the community. Vocational training, volunteer placement, employment, day treatment or continuing education are encouraged. Emphasis is on ADL and enhancing socialization skills. Specialized programs are also available for MICA, and low motivated residents.

Provides a comprehensive outpatient treatment program for individuals struggling with alcohol and drug addiction. Also does public awareness seminars about substance abuse and the resources of help available.

Provides a 10-bed facility with 24 hour oversight and support for adults with an Axis 1 of severe and persistent mental illness (SPMI) who are unable to live independently. Provides training and assistance in the skills of daily living.

Provides adults with a psychiatric disability ages 18 and over with a single room in a community residence that offers varying levels of supervision and assistance.

Offers assistance to participants to achieve goals of work, education, social connection, housing, community involvement, and health; overcoming the impact of psychiatric illness and disability that have obstructed these personal goals. Actively employs creative and expressive arts, symptom management strategies, adventure-based activities, peer support strategies, and cognitive remediation techniques to help people challenge personal barriers and rebuild their lives in recovery.

St. Joseph's Veterans and First Responders Residence is a fully credentialed addiction and trauma treatment program. Our 25-member community of Veterans and First Responders are served by 24/7, medical care; psychiatric nurse practitioner, primary counselors, spiritual counselors, mental health care manager.

Provides a 24 hour supervised residence for several adults with severe developmental disabilities. Services include medical and psychiatric care, basic living skills training, case management and physical, occupational and speech therapy.

The program's goal is for each person to function at the highest level possible in the least restrictive setting. Individuals in residential habilitation services is provided with the support to pursue his/her individualized interests and daily living skills training is offered in such areas as cooking, shopping, money management, laundry care and personal hygiene to maximize a person’s abilities. Active participation in daily life, including the pursuit of community inclusion, recreational, spiritual and social interests, is also fostered.

Committed to assisting clients to live meaningful, productive lives by providing flexible supports in a person-centered atmosphere. Residents of all programs learn the skills necessary to live independently. Treatment Apartments are certified by the New York State Office of Mental Health and provide consumers with the highest level of independence. Staff make routine visits and are available 24 hours a day in case of an emergency. Provides 12 one-bedroom apartments.

Supervised apartment building where residents with mental diagnoses receive training and support in all areas of daily living. Provides 24-hour staff supervision.

Transitional residential treatment program designed to facilitate movement to the resident's permanent housing choice. Some apartments are located at a main complex, where staffing offices are located for more intensive support and structure. Additional apartments are available throughout the Niagara Falls community for those needing less support. Double units are available on-site. Single and double units are available in community settings.

Rockland Hospital Guild provides residential programs for adults with hearing impairments and mental illness.

Provides 24 hour residential services to adults (ages 18+) who have a primary diagnosis of a intellectual disability. Residences are located in communities throughout Brookhaven Town. Services include recreational, direct-care, cook/housekeeper, medical, psychological and nutritional services, nursing, transportation and residential management services.

Provides 24 hour supervision at local community sites for adults with mental illness. Services provided at each site are restorative services, symptom management, community living skills, information and referral, crisis management, medication education and monitoring, and community integration. Staff provides supportive assistance and instruction in daily living skills and use of community resources. There are ten residences located in Nassau and Suffolk County. Transportation provided to local day treatment services.

OMH Certified Treatment Apartment Program. A staff person visits these apartments 3-5 times a week, relevant to provision of community rehabilitation skills. Staff are available 24 hours a day, 7 days a week.

Living arrangements are either in one of the agency run Community Residences or in a private home Family Care Residence.
